Abstract
We describe a case of subcapsular haematoma of the right lobe (SHL) and multifocal necrosis of the left lobe (MNL) of the liver following umbilical vein catheterisation (UVC) in a female at 24 weeks' gestation. SHL and MNL have been described as uncommon complications following umbilical vein catheterisation. To our knowledge this is the first time both complications have developed in the same patient. SHL should be considered in the differential diagnosis of hypovolaemic shock in this group of patients.
Résumé
Nous décrivons le cas d'un hématome sous-capsulaire du lobe droit (SHL) et d'une nécrose multifocale du lobe gauche (MNL) du foie suivant un cathétérisme ombilical (UVC) chez un prématuré de 24 semaines. SHL et MNL ont été décrits comme les complications inhabituelles suivant le cathétérisme des veines ombilicales. A notre connaissance, c'est la première fois que les deux complications ont été observées chez le même patient. SHL doit être considéré comme un diagnostic différentiel d'un choc hypovolémique dans ce groupe de patient.
Resumen
Describimos un caso de hematoma subcapsular el lóbulo derecho y necrosis multifocal del lóbulo izquierdo del hígado tras cateterización umbilical en una niña prematura de 24 semanas de gestación. El hematoma subcapsular y la necrosis multifocal se han descrito como complicaciones infrecuentes tras cateterización de la vena umbilical. A nuestro juicio esta es la primera vez en que ambas complicaciones se desarrollaron en el mismo paciente. El hematoma subcapsular debe ser considerado en el diagnóstico diferencial del shock hipovolémico en este grupo de pacientes.
Zusammenfassung
Der Fall eines weiblichen Frühgeborenen der 24. SSW mit subkapsulärem Hämatom des rechten Leberlappens (SLH) und multifokaler Nekrose des linken Leberlappens (MLN) nach Nabelvenenkatheterisierung wird vorgestellt. SLH und MLN werden hier das erste Mal in der Literatur als seltene Komplikationen einer NVK beschrieben. Beide Komplikationsformen müssen bei der Differentialdiagnose eines hypovolämischen Schocks in dieser Patientengruppe bedacht werden.
