Rare Injuries to the Distal Tibiofibular Joint in Children

Abstract

Injuries to the distal tibiofibular junction have been very precisely described in adults including diagnosis and treatment. By comparison, only two types of epiphyseal fracture were described as the equivalent of such an injury in the growing skeleton until now: the juvenile Tillaux fracture and the lateral triplane fracture. During a five-year period (1997 - 2001) twenty children with distal tibiofibular joint injury were treated in our department. Three previously unpublished types of epiphyseal injury to the distal tibial mortise are presented: intraepiphyseal fractures of Chaput's tubercle type 7A and 7B according to Ogden's classification (in two and one cases, respectively) and an intraepiphyseal subcortical tibial mortise disruption (one case). The authors suggest that these three types are transitive types between juvenile Tillaux fracture in children and disruption of the distal tibiofibular syndesmosis in adults. All three types were without significant displacement and were stable in stress positions of the ankle joint. Thus, the treatment was nonoperative with excellent results and without sequelae.

Les traumatismes de la jonction tibio-péronière ont été très précisément décrits chez l'adulte en ce qui concerne leurs diagnostics et traitement. Par ailleurs, seulement deux types de fractures épiphysaires ont été décrits comme un équivalent de tel traumatisme chez le squelette en croissance: les fractures juvéniles de Tillaux et les fractures latérales Triplane. Durant une période de 5 ans (1997 - 2001), 20 enfants présentant un traumatisme de l'articulation tibio-péronière distale étaient observés: une fracture intra-épiphysaire de la mortaise tibiale distale une fracture intra-épiphysaire du tubercule de Chaput de type 7 A correspondant au type 7 B de la classification de Ogden (dans deux cas) et une disjonction de la mortaise tibiale (dans un cas). Les auteurs supposent que ces trois types sont des types intermédiaires entre la fracture juvénile de Tillaux et la disjonction de l'articulation tibio-péronière distale de l'adulte. Tous ces types étaient sans déplacement significatif et stables. Le traitement non opératoire donne d'excellents résultats sans séquelle.

Las lesiones de la unión distal tibiofibular han sido descritas con precisión en adultos incluyendo su diagnóstico y tratamiento. Por otra parte solo han sido descritos hasta ahora dos tipos de fractura epifisaria como equivalentes de tales lesiones en el esqueleto en crecimiento: Las fracturas juveniles de Tillaux y las fracturas laterales triplano. Durante el periodo de 5 años 1997 - 2001 hemos tratado 20 niños con lesiones de la articulación tibiofibular distal que se presentaron como fracturas intraepifisarias: fracturas intraepifisarias del tubérculo de Chaput de tipo 7 A y 7 B de acuerdo con la clasificación Ogden (un caso respectivamente) y rotura intraepifisaria subcortical de la mortaja tibial en un caso. Los autores suponen que estos tres tipos son de transición entre la fractura juvenil de Tillaux y la rotura de la sindesmosis distal tibiofibular en adultos. Los tres tipos no tenían desplazamiento significativo y eran estables en posiciones de fuerza de la articulación del tobillo. Por lo tanto el tratamiento fue no operatorio con excelentes resultados y sin secuelas.

Zusammenfassung

Verletzungen des distalen Tibiofibulargelenkes sind bei Erwachsenen sehr genau beschrieben worden, und zwar sowohl die Diagnose wie die Behandlung. Auf der anderen Seite sind im Kindesalter nur zwei Formen der Epiphysenfraktur bekannt geworden als equivalente Verletzungen des wachsenden Skeletts gegenüber dem Erwachsenen: zum einen die juvenile Tillaux-Fraktur, zum anderen die laterale Triplane-Fraktur. Im Fünfjahreszeitraum von 1997 bis 2001 wurde bei 20 Kindern eine distale tibiofibulare Verletzung behandelt. Dabei zeigten sich drei bisher nicht publizierte Formen einer Epiphysenverletzung: die intraepiphyseale Fraktur des Tuberculum vom Typ 7A respektive 7B nach der Klassifikation von Ogden, in zwei respektive einem Fall, und die intraepiphyseale subkortikale Tibiarinne-Ruptur in einem Fall. Die Autoren vermuten, dass diese Frakturtypen Zwischenformen zwischen der juvenilen Tillaux-Fraktur im Kindesalter und einer Ruptur der distalen tibiofibularen Syndesmose bei Erwachsenen darstellen. Alle drei Formen zeigten keine signifikante Dislokation und eine stabile Stressposition des Sprunggelenks. Daher war die Behandlung in allen Fällen nicht operativ und führte zu ausgezeichneten Ergebnissen ohne Spätbeschwerden.
