Das ökologisch-therapeutische Milieu in der stationären Behandlung

Zusammenfassung

Anliegen: Bei der behandlungsförderlichen Gestaltung eines therapeutischen Milieus spielen neben interaktionalen Gesichtspunkten auch Aspekte des ökologischen Milieus eine wichtige Rolle. Es wird ein ausführliches Review des wissenschaftlichen Kenntnisstandes zum ökologischen therapeutischen Milieu gegeben und der „Wohnmilieufragebogen” vorgestellt, der wichtige Aspekte des ökologischen Milieus operationalisiert und zu messen erlaubt. Methode: 127 Patienten zweier psychosomatischer Rehabilitationskliniken wurden mit dem Wohnmilieufragebogen um ihre Beurteilung des häuslichen Wohnmilieus und des Klinikmilieus gebeten. Ergebnisse: Die Ergebnisse zeigen, dass sowohl das Wohn- als auch das Klinikmilieu insgesamt positiv beurteilt werden. Im Vergleich von Wohn- und Klinikmilieu ergeben sich einige signifikante Unterschiede (in der Klinik mehr Platz, weniger Lärm, besserer baulicher Zustand, mehr Ruhe und Konfliktfreiheit im Zusammenleben; zu Hause größere Sauberkeit, bessere Verfügbarkeit von Hilfsmitteln, mehr Möglichkeit eigenen Interessen zu folgen). Der Fragebogen hat eine gute Retestreliabilität (Wohnmilieu r = 0,895; Klinikmilieu r = 0,854) und eine hohe interne Konsistenz (Cronbachs α Wohnmilieu 0,82; Klinikmilieu 0,83), sodass der Summenscore sinnvoll zu interpretieren ist. Schlussfolgerung: Die vorgelegte Literaturübersicht zeigt, dass das ökologische Milieu ein wichtiger Teilaspekt des therapeutischen Milieus ist und mehr Aufmerksamkeit in Forschung und Praxis erhalten sollte. Der Wohnmilieufragebogen erlaubt eine Operationalisierung und Beurteilung des ökologischen Milieus und hat sich als ein reliables und ökonomisches Instrument erwiesen. Unterschiede in der Beurteilung des Wohn- und Klinikmilieus sprechen für die Sensitivität bezüglich unterschiedlicher Anwendungsbereiche. Zukünftig kann der Fragebogen zur Qualitätssicherung und Verbesserung therapeutischer Milieus genutzt werden.

Abstract

Objective: Therapeutic milieus can be characterized by interactional qualities („social atmosphere”) and by ecological conditions. The paper gives a literature review on ecological aspects of the therapeutic milieu. Furthermore, a questionnaire is introduced operationalizing and measuring the quality of ecological milieus. Method: 127 patients of two psychosomatic inpatient units were asked to evaluate the ecological milieu at home and in the hospital, using the housing milieu questionnaire. Results: Patients rated both the quality of their home and of the hospital-milieu rather positive. In a comparison of both milieus the patients rated the hospital better in respect to space availability, noise, building status, and conflicts with others, while the home milieu gets higher ratings for cleanliness, the availability of household devices, and the possibility to pursue own interests. The housing milieu questionnaire has a high internal consistency (home: Cronbach's α = 0.82, clinic: Cronbach's α = 0.83) and good retest reliability (home: r = 0.895; clinic: r = 0.854). Conclusions: The literature review shows that the ecological milieu is an important part of the therapeutic milieu and should get more attention in science and practice. The housing milieu questionnaire allows to describe ecological aspects of the therapeutic milieu. Differences between home and hospital argue for the sensitivity of the scale. The questionnaire can be used to analyze the quality of ecological aspects of the therapeutic milieu.
