Ein 46-jähriger Patient wurde uns wegen einer seit etwa einem Jahr bestehenden chronischen gastrointestinalen Blutung unklarer Lokalisation zugewiesen. Mehrere Ösophago-Gastro-Duodenoskopien, zwei Koloskopien sowie eine Push-Enteroskopie waren unauffällig gewesen. Bei normalem körperlichen Untersuchungsbefund fand sich in der Abdomensonographie das in der [Abb. 1] gezeigte Bild.

Fragen:

Welche pathologische Veränderung ist erkennbar? Welche Differenzialdiagnosen kommen hier in Betracht? Welche ergänzenden Untersuchungen sollten erfolgen?
