Zusammenfassung
Neun Patienten mit der duplcxsonogra-phischen Diagnose einer zervikalcn Raumforderung wurden mittels farbkodierter Dopplersonographie vergleichend untersucht. Von 3 duplexsonographisch diagnostizierten Aneurysmen der Arteria carotis interna konnte farbdopplersonographisch nur eines bestätigt werden. Bei den beiden übrigen Patienten fand sich ein 360° Coiling der A. carotis interna. Von 3 echoarmen Läsionen, die in der Duplexsonographie als Lymphomc interpretiert wurden, konnte in der Farbdopplersonographie in 2 Fällen ein großteils thrombosiertes Carotis-interna-Aneurysma, bei einem Patienten ein Glomus-caroticum-Tumor differenziert werden. Die duplexsonographische Diagnose eines Glomustumors und von zwei AV-Malformationcn wurden in der Farbdopplersonographie bestätigt. Eine ergänzend durchgeführte Angiographie verifizierte die farbdopplersonographisch gestellten Diagnosen in 7 Fällen. Bei den übrigen 2 Patienten mit Gefäßverlaufsabnormitäten wurde auf eine Angiographie verzichtet. Die farbko-dierte Dopplersonographie war somit in der Beurteilung vaskulärer Raumforderungen des extrakraniellen Karo-tisstromgebietes der konventionellen Duplexsonographie überlegen und lieferte wesentliche Zusatzinformationen.
Abstract
Ninc patients with a cervical tumour, dia-gnosed by duplex Dopplcr sonography were additionally evaluated by colour-coded Dopplcr sonography (CCDS). The duplex Dopplcr diagnosis of three internal carotid artery aneurysm was confirmed by CCDS in only one patient. In the remaining two cases CCDS identified a marked coiling of the internal carotid artery. Three low echogenic lesions, interpreted as cervical lymphomas by conventional Doppier sonography were identified as two internal carotid aneurysm with partial thrombosis and a carotid body tumour in the third patient by CCDS. Du-plex-Doppler sonography and CCDS agreed in the diagnosis of two AV-malformations and one carotid body tumour. Angiography confirmed the CCDS-diagnosis in 7 patients. In the two patients with coiling of the ICA angiographie evaluation was not performed. Thus, CCDS was superior to duplex Doppier sonography in the evaluation of vascular lesions of the extracranial section of the carotid artery and provided additional diagnostic infor-mation.
