Zusammenfassung
Echofreie Bezirke der Plazenta können drei verschiedene anatomische Substrate als Ursache haben. Einmal in der Folge einer fortgeschrittenen Verkalkung des Organs auftretende ringförmige Eingrenzungen von Cotyledonen. Im Innern dieser echostarken Ringe kommt es durch Schallauslöschung zur Aufhebung der typischen gekörnten Plazentastruktur.
Davon läßt sich das seltenere Phänomen eines nicht durch einen echodichten Ring eingekreisten echoskopischen Loches abgrenzen. Das kann einmal durch eine avillöse blutgefüllte Ausweitung des chorioepithelialen Zwischenraumes hervorgerufen werden, ein Befund ohne pathognomonische Bedeutung. Weitgehend echoskopisch ident kann aber auch ein kolliquierter Infarkt der Plazenta sein, der sehr wohl klinische Bedeutung haben würde.
Es ist anzunehmen, daß sonographisch allein die Unterscheidung prospektiv nur schwierig zu treffen sein wird.
Abstract
Echo-free regions of the placenta can be due to three different anatomical Substrates: First of all, ring-shaped contours of cotyledones may occur as a result of progressive calcification of the organ; these rings produce a pronounced echo, so that the typical grained structure of the placenta is no longer recognizable due to sound extinction.
Secondly, there may be an echoscopic “hole” not surrounded by a ring which is impermeable to echo; this phenomenon, however, is relatively rare. It may be due, on the one hand, to an avillous blood-filled expansion of the chorioepithelial interstitial space without pathognomonic significance, but it can also be confused with an echoscopically largely identical colliquated infarction of the placenta which could well be of clinical importance.
Sonography alone will probalby not produce sufficiently conclusive evidence and must be supported by other diagnostic procedures.
