Zusammenfassung
Im Gegensatz zu herkömmlichen diagnostischen Verfahren bietet die intrakavitäre Sonographie erhebliche Vortneile bei der T-Klassifizerung von Harnblasentumoren. Neben der guten Praktikabilitäl ist das hohe Maß an Übereinstimmung mit den pathohistologischen Befunden nach Operation hervorzuheben. Der Einsatz bei 97 Patienten mit urothelialem Karzinom der Blase hatte eine Trefferquote von 63%. Mögliche Ursachen für ein Overstaging in 32% der Fälle werden an Beispielen erläutert. Ein Teil der sonographischen Fehl-Manipulationen (gain-Verstellung) sowie mit wachsander Erfahrung vermieden werden. Insbesondere sind benigne Blesenerkrankungen zu berücksichtigen, die sonographisch als Malignom imponieren. Diese Fälle unterstreichen die Notwendigkeit, den Ultraschallbefund jeweils mit einar Histologie zu korrelieren.
Abstract
Intracavitary sonography proved to be advantageous with regard to the T-classification of urinary bladder tumours in comparison with conventional diagnostic methods. Besides the good practicability, the high degree of correspondence between sonography and histopathological findings is emphasized. In 97 patients wiih urothelial bladder cancer this method yielded 63% correct results. In 32% overstaging was noticad, possible factors of which were exemplified. Part of the sonographic failures can be avoided by means of tachnical manipulation (gain-modification) togethar with increasing experrience of the examiner. In particular benign bladder disease, which can be sonographically confused with cancer, has to be considered. These cases indicate the need to correlate particular sonographic findings with histopathology.
