Zusammenfassung
Unter den multimodal evozierten Potentialen haben besonders die durch Magnetstimulation motorisch evozierten Potentiale (MEP) und die somatosensiblen evozierten Potentiale (SEP) neue Einsatzbereiche hinzugewonnen. Bei Hirninfarkten lassen sich hiermit bereits in den ersten Tagen Rückschlüsse auf den vermutlichen Verlauf ziehen, und bei der globalen zerebralen Hypoxie nach Reanimation können Patienten mit infauster Prognose frühzeitig auf eine Minimaltherapie zurückgestuft werden. Durch kortikale Magnetstimulation mit variablen Techniken ergaben sich interessante Einblicke in veränderte exzitatorische und inhibitorische Mechanismen bei diversen Dystonieformen. Schließlich erwiesen sich MEP- und SEP-Untersuchungen als wertvolle diagnostische und prognostische Hilfe bei zervikalen Myelopathien, insbesondere bei diskrepanten Ergebnissen der klinischen und bildgebenden Diagnostik.
Summary
Evoked potentials are valuable tools in the diagnosis and prognosis of a variety of neurological disorders. In this review, new clinical applications of motor evoked potentials (MEP) after transcranial magnetic stimulation and somatosensory evoked potentials (SEP) are discussed. Early-performed MEP and SEP seem to be helpful in predicting the functional recovery from stroke. In patients with anoxic coma, an early prognosis can be established by well-defined EP-criteria. Transcranial magnetic stimulation provides insights into abnormal cortical motor excitation and inhibition in focal, task specific dystonia. Finally MEP and SEP are useful diagnostic tools in cervical spondylotic myelopathy, particularly when clinical and radiological findings are contradictory.
