Zusammenfassung
Eine Kombinationsschädigung von N. ulnaris und N. medianus im Bereich des Oberarms kann Leitsymptom eines traumatischen Aneurysmas der A. brachialis sein. Die vorliegende Fallbeschreibung eines Aneurysmas vier Monate nach Osteosynthese einer Humerusschaftfraktur zeigt in der Verlaufskontrolle zwei Monate nach Resektion des Aneurysmas für den N. medianus eine nahezu vollständige Wiederherstellung der Funktion, für den N. ulnaris einen kompletten Funktionsausfall. Frühzeitige Diagnostik und schnelle operative Revision sollten daher in interdisziplinärer Zusammenarbeit von Neuro- und Gefäßchirurgen angestrebt werden.
Summary
A combined paralysis of the ulnar and the median nerve located at the upper arm can be the leading clinical finding of a traumatic aneurysm of the brachial artery. This case report describes an aneurysm which developed four months after osteosynthesis of a humeral bone fracture. Neurological and electrophysiological testing eight weeks after resection of the aneurysm revealed gradual improvement of median but not of ulnar nerve functions. Therefore, early diagnostic steps and operative intervention are suggested in the case of clinical suspicion of nerve compression caused by an aneurysm.
