Endokrinologische Diagnostik bei Hypophysentumoren

Zusammenfassung

Endokrinologische Diagnostik bei einem Verdacht auf einen Hypophysentumor dient zwei Aufgaben: erstens dem Nachweis eines Hormonüberschusses und zweitens dem Ausschluß einer Hormonmangelsituation. Auf der Basis eines solchen differenzierten Vorgehens werden rationale Methoden zur Diagnostik von Akromegalie (Wachstumshormon-), Hyperprolactinämie (Prolactin-) und Morbus Cushing) (ACTH-Überschuß) dargestellt und eine praktikable Strategie zum Nachweis einer Hypophysenvorderlappeninsuffizienz aufgezeigt.

Summary

In cases of suspected pituitary adenoma endokrine diagnostics is called upon to answer two questions:

• - is there a hormonal excess?

• - is there a hormone deficiency, i.e. an anterior pituitary failure?

On the basis of these two questions a rational method for the diagnosis of acromegaly (growth hormone excess), hyperprolactinaemia (prolactin excess) and Cushing's syndrome (ACTH excess) is combined with an easily performed strategy for the detection of anterior pituitary failure (complete or incomplete)
