Anatomische Variationen des R. thenaris nervi mediani aufgrund von 237 Operationsbefunden

Zusammenfassung

In 42 Fällen von insgesamt 237 Canalis carpi-Freilegungen beobachteten die Verfasser anatomische Variationen des R. thenaris, was einer Frequenz von 17,7% entspricht. Die Variationen waren: in 9 Fällen (3,8%) ein doppelter R. thenaris, bei einem Patienten ein dreigeteilter R. thenaris (0,4%), an 2 Händen entsprang der R. thenaris ulnar vom Hauptast des N. medianus (0,8%); viermal wurde ein palmarer Ursprung des R. thenaris beobachtet (1,7%) und bei 26 Patienten (10,8%) durchbohrte der R. thenaris das Lig. carpi transversum.

Von diesen Variationen sind in der Weltliteratur bisher der dreigeteilte R. thenaris und ein palmarer Ursprung nicht mitgeteilt worden. Die Autoren weisen auf die chirurgische bzw. neurologische Bedeutung der einzelnen Variationen hin.

Summary

In 42 out of a total of 237 cases of surgery of the wrist laying open the carpal tunnel, the authors observed anatomical variations of the thenar branch of the median nerve. This represents a percentage frequency of 17.7%. The variations were: In 9 cases (3.8%) a doubled thenar branch; in one patient a thenar branch divided into three parts (0.4%); with two hands the thenar motor branch branched off ulnar from the main branch of the median nerve (0.8%); in four cases palmar origin of the thenar branch was seen (1.7%); and in 26 patients the thenar branch penetrated the ligamentum carpi transversum (retinaculum flexorummanus) (10.8%).

Of these variations, there has so far not been any report in literature on the tripartite thenar branch and on a palmar origin. In connection with their observations the authors point out the surgical and/or neurological importance of the individual variations.
