Intradurales spinales Lipom mit medullärer Ausbreitung

Zusammenfassung

15jähriger Junge mit seit 8 Monaten fortschreitenden Sensibilitätsstörungen und schlaffen Paresen der oberen Extremitäten, bei dem 4 Jahre zuvor ein subkutanes Lipom zwischen den Schulterblättern operativ entfernt worden war. Neurologisch bestand ein zerviko-thorakales medulläres Syndrom. Mit Hilfe der spinalen Computer-Tomographie konnte die intra- und extramedulläre Tumorausbreitung und das in Reststrängen aufgeteilte Rückenmark abgegrenzt werden. Die Artdiagnose wurde anhand des Computer-Tomogramms densitometrisch bestimmt.

Summary

The article reports on a 15-year old boy with disturbances of sensitivity which have been progressing for the last 8 months, and atonic pareses of the upper extremities. Four years previously, a subcutaneous lipoma between the shoulder blades had been removed by surgery. Neurologically speaking, the condition of the boy was characterised by a cervico-thoracic medullary syndrome. It was possible to define the intramedullary and extramedullar spread of the tumour, as well as the residual cord formation of the medulla spinalis, by employing spinal computerised tomography. Differential diagnosis was effected densitometrically by means of the computer tomogram.
