Zusammenfassung
Die Polyneuropathie ist eine sehr häufige Komplikation des Diabetes mellitus, die mitbestimmend für die Lebenserwartung und Lebensqualität des Diabetikers ist. Die autonome diabetische Neuropathie (ADN) wurde im Vergleich zu der peripheren sensomotorischen Neuropathie weniger beachtet. Ihre vielfältige Symptomatik wurde eher kasuistisch betrachtet und nicht als Ausdruck einer Systemerkrankung begriffen. Der frühzeitigen und sicheren Diagnose der ADN kommt jedoch eine große prognostische Bedeutung zu.
In den letzten Jahren sind verschiedene Untersuchungsmethoden zur quantifizierbaren Erfassung der ADN entwickelt worden (z. B. Analyse der Herzfrequenzvariation, Orthostaseuntersuchungen nach passivem Aufrichten, Infrarot-Pupillometrie, maximales Blasenvolumen etc.). Ein neu entwickelter Kleincomputer zur automatischen Auswertung der Herzfrequenzschwankungen unter verschiedenen Bedingungen erlaubt eine einfache routinemäßige und quantitative Untersuchung der ADN am Herzen.
Summary
Polyneuropathies are frequent complications of diabetes mellitus which determine the quality of life and the life expectancy of the diabetic patient. Until recently autonomous diabetic neuropathy has attracted less interest than peripheral polyneuropathy, and its implications as a systemic disorder were little recognised. However, autonomous diabetic neuropathy (ADN) is of major prognostic importance.
During the last few years several diagnostic procedures have been evaluated which allow a quantitative determination of ADN (e.g., beat-to-beat variation analysis; postural hypotension and heart rate variation; pupil-lometry; maximal vesical volume; and others). A recently developed minicomputer allows a simple and rapid analysis and quantification of ADN of the heart by autonomous heart rate variation analysis.
