Thromboserisikomanagement bei internistischen Patienten - Welche Patienten benötigen eine medikamentöse Prophylaxe?

Trotz zahlreicher Empfehlungen zur Thromboseprophylaxe werden viele nicht-chirurgische (internistische) Patienten nicht adäquat versorgt. Dies liegt sowohl an der Vakanz einer nationalen Leitlinie, als auch an der geringen Anzahl von in der täglichen Praxis validierten Risikomodellen. Die hier beschriebenen drei Risikostratifizierungen sollen dem Arzt in der täglichen Praxis bei der Entscheidung helfen, welcher nicht-chirurgische Patient einer medikamentösen Thromboseprophylaxe bedarf. Eine Stratifizierung zwischen einem thromboembolischen Niedrig- und Hochrisikobereich bei nicht-chirurgischen Patienten ist zu bevorzugen. Ein hohes thromboembolisches Risiko erfordert eine medikamentöse Thromboseprophylaxe bei nicht-chirurgischen Patienten. Diese sollte mit einem Heparin in der Dosis des Hochrisikobereiches vorgenommen werden. Hierbei sollten niedermolekulare Heparine (NMH) zur medikamentösen Thromboseprophylaxe bevorzugt eingesetzt werden. Ein niedriges Risiko bei nicht-chirurgischen Patienten verlangt keine medikamentöse Thromboseprophylaxe.

Adverse of consensus-group recommendations hospitalized patients with acute medical conditions may not receive appropriate thromboprophylaxis. The reasons are the lack of a national guideline and the poor number of in the daily clinic praxis validated risk-assessment models. The following three presented risk strategies should support to come to the right decision, which medical patient need a medical thromboprophylaxis. A differentiation in a low and a high thromboembolic risk of medical patients is to prefer. A medical patient with a high thromboembolic risk need a medical thromboprophylaxis. The heparins are the favourites of the medical thromboprophylaxis. Low molecular weight heparins (LMWH) in a high-risk dose have the best evidence. A medical patient with a low thromboembolic risk need no medical thromboprophylaxis.
