Imitation einer primär sklerosierenden Cholangitis durch eine Langerhans-Zell-Histiozytose

Zusammenfassung

Wir berichten über einen jungen Patienten, der zur Klärung eines cholestatischen Ikterus stationär aufgenommen wurde. Die endoskopische Diagnostik ergab in der endoskopisch retrograden Cholangiopancreaticografie (ERCP) für eine primär sklerosierende Cholangitis (PSC) typische Läsionen der intrahepatischen Gallenwege. Eine eigenständige Gallengangserkrankung konnte jedoch ausgeschlossen werden. Vielmehr ließen sich als Ursache der biliären Veränderungen Infiltrate von CD1a-positiven Histiozyten in der Leber im Rahmen einer Langerhans-Zell-Histiozytose mit zusätzlicher Manifestation in Lunge und Milz diagnostizieren. Bei der zytologischen Knochenmarksuntersuchung fand sich außerdem der Befund einer chronisch myelo-monozytären Leukämie, während Histiozyten im Knochenmark nicht nachgewiesen werden konnten.

Abstract

We report an adult man who was hospitalized for evaluation of an icterus. The diagnostic work-up by endoscopic retrograde cholangiopancreaticography (ERCP) revealed lesions of the intrahepatic bile ducts typical of primary sclerosing cholangitis (PSC). Whereas a specific biliary injury has been excluded further investigation revealed an infiltration of CD1a positive histiocytes in the liver within the scope of Langerhans cell histiocytosis, also involving lung and spleen, causing the alterations of the bile ducts. Cytological investigation of bone marrow aspiration additionally revealed the diagnosis of myelo-monocytic leukaemia, histiocytes could not be detected.
