Hypoglobus Due to Tumours in the Superior Orbital Region

Zusammenfassung

Hintergrund: Tumore im superioren Orbitabereich können zu einem Hypoglobus führen. Wir vergleichen die prä- und postoperative Bulbusstellung bei zwei Patienten mit unterschiedlichen Tumorarten. Anamnese und Befund: Ein 29-Jähriger und eine 58-jährige Patientin stellten sich mit einem ausgeprägten Hypoglobus vor. Bei beiden Patienten waren Ausmaß und zeitliches Bestehen des Hypoglobus vergleichbar. Radiologisch wurde beim jüngeren Patienten ein pleomorphes Adenom vermutet, bei der älteren Patientin ein kavernöses Hämangiom. Therapie und Verlauf: Intraoperativ bestätigten sich die radiologischen Verdachtsdiagnosen. Beide Läsionen konnten vollständig exzidiert werden. Beim 29-jährigen Adenompatienten erholte sich der zuvor reduzierte Visus vollständig, ein residueller Hypoglobus blieb jedoch selbst nach 10 Monaten bestehen. Im Gegensatz dazu kam es bei der 58-jährigen Patientin mit dem Hämangiom zu einer vollständigen Regredienz des zuvor langbestehenden Hypoglobus. Schlussfolgerung: Die histologische Beschaffenheit eines orbitalen Tumors im superioren Orbitabereich ist für die Reversibilität des Hypoglobus von Bedeutung.

Abstract

Background: Tumours in the superior orbital region can cause hypoglobus. We compare the pre- and postoperative globe position in two patients with different tumour types. History and Signs: A 29-year-old man and a 58-year-old woman presented with pronounced hypoglobus. In both patients time-scale and importance of the lesion were comparable. Imaging suggested a pleomorphic adenoma in the younger patient, whereas the female patient was radiologically diagnosed with a hemangioma. Therapy and Outcome: Both radiological diagnoses could be histologically proven. Both lesions were excised in toto. The adenoma patient showed a full recovery of the preoperatively reduced vision. However, the hypoglobus persisted even after ten months. The hemangioma patient had an immediate full recovery of her hypoglobus. Conclusion: The histological type of a tumor in the superior orbit is of importance regarding the degree of reversibility of the accompanying hypoglobus.
