Abstract
Aiming at testing the hypothesis that, in the foetus with gastroschisis (GX) peritoneal dialysis during antenatal bowel exposure to the amniotic fluid may induce some modifications of the internal environment, we have studied the composition of blood, amniotic fluid and allantoic fluid in a chick embryo model of GX. Intestinal loops were surgically eviscerated in 184 embryos on the 14th day of incubation and exposed to the mixture of both embryonal fluids until the 19th day (hatching expected at the 21st). Typical lesions of GX were present in half of the 94 survivors (n = 42). For comparison we used a control group of non-manipulated animals (n = 19) and a "sham" group of animals in which the operation was carried out only until umbilical manipulation but without bowel exposure (n = 22).
Chicks with GX were smaller and slightly dehydrated as attested by clinical signs and had moderately increased hematocrit. They had significant hyperkaliaemia and slight decreases in serum Na and CI. Urea, total protein and protein fractions were not modified.
Taking into account the gradients for such substances existing between the dialysate (amnio-allantoic mixture) and the embryonal blood, the changes observed, albeit slight, suggest that there was some evidence of dialysis through the peritoneum: lower Na and CI concentrations in the dialysate than in blood, induced slight falls in serum Na and CI whereas higher K concentration resulted in severe hyperkaliemia. The high levels of albumin in the chick amniotic fluid did not allow protein depletion and therefore total protein and its fractions remained unchanged. Serum urea was, contrary to our expectations, insignificantly elevated in spite of the high levels of this solute in the dialysate at the end of the experiments.
Since some clinical features of human GX (low birth weight, hypoproteinemia) may be related in part to this process of prolonged dialysis, some transamniotic modification of fetal internal environment could be tried in experimental models of the disease in an attempt to provide some alternatives for optimizing peri-natal management.
Zusammenfassung
Um die Hypothese zu überprüfen, daß bei Feten mit Gastroschisis (GX) die pränatale Lage des Darmes in der Amnionflüssigkeit und die simultane Peritonealdialyse des Fetalabdomens mit Amnion/Allantoin-FIüssigkeit zu Veränderungen an Darm und Peritoneum führt, werden an Hühnerembryonen mit GX Blut, Amnionflüssigkeit und Allantoin untersucht. Die Darmschlingen des Embryons werden am 14. Intubationstag bei 184 Embryonen operativ eviszeriert und bis zum 19. Tag einer Mischung beider embryonaler Flüssigkeiten ausgesetzt. Typische gastroschisisanaloge Veränderungen wurden bei 42 von 94 Embryonen gefunden. Als Kontrolle dienten 19 nicht und 22 schon operierte Tiere. Hühnchen mit GX waren kleiner und wirkten klinisch leicht dehydriert. Der Hämatokrit war leicht erhöht; sie zeigten eine signifikante Hyperkaliämie und einen leichten Abfall von Serum Na und Chlor. Harnstoff, Gesamteiweiß und Eiweißtraktionen waren leicht verändert.
Im Hinblick auf den Gradienten für die untersuchten Substanzen zwischen dem Analysat (Amnion/Allantoinflüssigkeit) und dem embryonalen Blut lassen die beobachteten Blutveränderungen eine Dialyse durch das Peritoneum vermuten. Hierfür sprechen niedrigere Na- und Cl- Konzentrationen im Dialysat als im Blut und der leichte Abfall von Natrium und Chlor im Serum bei gleichzeitig hoher Kalium-Konzentration mit Hyperkaliämie. Die hohen Albuminspiegel in der Amnionflüssigkeit erlauben keine Wanderung der Albuminmoleküle, weshalb das Gesamteiweiß und die Eiweißfraktionen im Serum unverändert blieben. Entgegen unseren Erwartungen war die Serumkonzentration von Harnstoff nicht erhöht, trotz hoher Spiegel im Dialysat am Ende des Experiments.
Aus den Untersuchungen kann gefolgert werden, daß möglicherweise einige Symptome der menschlichen Gastroschisis wie niedriges Geburtsgewicht und Hypoproteinämie auf eine verlängerte Peritonealdialyse zurückzuführen sind. Aus diesem Grunde können möglicherweise transamniotische Beeinflussung des fetalen intraperitonealen Milieus im Rahmen weiterer Experimente die Bedingungen für die peritoneale Behandlung der Gastroschisis verbessern helfen.
