Inflammatory Bowel Disease in Childhood Presentation and Diagnosis

Abstract

150 patients whose first symptoms of inflammatory bowel disease (IBD) presented before puberty were seen at this hospital between 1955 and 1979. Ulcerative colitis with the rectum less severely involved than the colon was commoner than usually reported in adults. Colectomy with ileorectal anastomosis allowed these children to go through school life and adolescence without an ileostomy. Failure of medical treatment was much commoner in those presenting with already total colonic involvement. Children with Crohn's disease were twice as likely to come to surgery within the first five years of illness than those with ulcerative colitis. Delayed diagnosis (mean 2.6 years) was much greater in Crohn's disease, especially in the 25% presenting initially with failure to thrive and no specifically alimentary symptoms.

Zusammenfassung

Am Hospital for Sick Children wurden im Zeitraum von 1955 bis 1979 150 Patienten mit entzündlichen Darmerkrankungen wie Morbus Crohn und Colitis ulcerosa beobachtet. Die Colitis ulcerosa, bei der das Rektum weniger schwer betroffen war als das Kolon, wurden häufiger beobachtet als bisher vom Erwachsenen angenommen wurde. Eine Kolektomie mit ileorektaler Anastomose erlaubte diesen Kindern ein normales Leben mit normalem Schulbesuch ohne Ileostoma. Ein Versagen der konservativen Therapie war um so häufiger bei den Patienten, die bereits einen vollständigen Kolonbefall aufwiesen. Kinder mit einem Morbus Crohn wurden zweimal so häufig innerhalb der ersten fünf Jahre ihrer Erkrankung operiert als Patienten mit Colitis ulcerosa. Beim Morbus Crohn wurde die Diagnose wesentlich später (durchschnittlich 2,6 Jahre nach Krankheitsbeginn) gestellt, wobei sich bei 25% der Kinder ein Minderwuchs ohne spezifische gastrointestinale Symptome zeigte.
