Akute Armschwäche aus neurologischer Sicht – Erste Lokalisation der zugrunde liegenden Läsion

Bei akut auftretender Armschwäche sind eine Vielzahl unterschiedlicher Läsionsorte und Läsionsarten zu bedenken, die neben einer ausführlichen Anamnese und klinisch-neurologischen Untersuchung in der Regel laborchemische, elektrophysiologische und bildgebende Zusatzuntersuchungen erfordern. Sinnvoll ist eine Einteilung der klinischen Syndrome nach Lokalisationsort, also zentral (kortikal, subkortikal, Rückenmark) beziehungsweise peripher (Wurzel, Plexus, peripherer Nerv, Endplatte, Muskel). Oft helfen akzessorische Symptome wie Schmerzen, sensible Defizite und Befall anderer Körperregionen (Gesicht, Beine), eine erste klinische Zuordnung zu treffen und zielführende Zusatzuntersuchungen zu veranlassen.

In the patient with new onset arm weakness the underlying lesion may be distributed over a variety of central and peripheral nervous system locations, and may be due to a variety of pathogenic processes. A detailed history and physical examination are required to select appropriate ancillary testing to make the correct diagnosis. The main causes of acute brachial paresis are reviewed in this report. For the clinical diagnostic process it is useful to classify brachial weakness due to central lesions (cortical, subcortical, spinal cord) and due to peripheral lesions (nerve roots, plexus, peripheral nerve, neuromuscular endplate, muscle). Accessory symptoms such as accompanying pain, sensory deficits, involvement of other body regions (face, legs, trunk) are important to clinically localize the lesion and obtain appropriate confirmatory tests.
