Zur Differentialindikation der Kurorttherapie bei entzündlich rheumatischen Erkrankungen - 1 . Mitteilung Indikationsprofil der Rheumatoid Arthritis für die antiphlogistische Wirkung der Moorkurorttherapie

Zusammenfassung

Die Bedeutung der Kuren als medizinisches Versorgungssystem bei der Betreuung von Patienten mit RA sowie ihr beachtenswerter ökonomischer Aufwand verpflichten zur ständigen Vervollkommnung dieser Therapie, wobei vor allem eine Qualitätsverbesserung vordringlich erscheint. Die vorliegende Arbeit ist ein Beitrag hierzu. Es wurde versucht, für standardisierte Kuren mit Moorpackungen, Moorbädern und Moorpasteanwendungen als balneologische Grundbehandlungen eine Differentialindikation der RA zu begründen. Die Einschätzung des Therapieeffektes erfolgte durch Beurteilung der Entzündungsaktivität nach einem für die Effektivitätsbeurteilung bei RA inaugurierten Verfahren. Die Klassifizierung des Krankheitsbildes der RA erfolgte nach dem radiomorphologischen Stadium, der radiomorphologischen Progredienz, der Entzündungsaktivität, der Gelenkfunktionsminderung und der Medikation. Es wird gezeigt, dass durch eine differenzierte Wertung der Entzündungsaktivität am klassifizierten Krankheitsbild Aussagen für eine Differentialindikation der RA zur Moorkurorttherapie möglich werden. Der Therapiewert der untersuchten Kuren ist für die verschiedenen Merkmalgruppen unterschiedlich. Die Entzündungsaktivität ändert sich im Verlauf der Kur sowohl in Beziehung zu den Klassifikationsmerkmalen als auch in Abhängigkeit von den balneologischen Grundbehandlungen und der Kurdauer. Es wird ein Indikationsverzeichnis vorgestellt, das praxisrelevante Effektivitätskennziffern ausweist.

Summary

The importance of cures as a system of medical care for patients with rheumatoid arthritis as well as their remarkable economic expenditure oblige to a permanent improvement of this therapy, in which case above all an amelioration of the quality appears as most important. The present paper is a contribution to this. It was tried to give reasons for a differential indication of the rheumatoid arthritis as to standardized cures with mud packs, mud baths and applications of mud paste as balneologic basic therapies. The assessment of the therapeutic effect was done by the judgement of the activity of inflammation according to a method inaugurated for the judgement of the effectiveness in rheumatoid arthritis. The classification of the clinical picture of rheumatoid arthritis was performed according to the radiomorphological stage, the radiomorphological progression, the activity of inflammation, the reduction of the function of the joints and the medication. It is indicated that by a differentiated valuation of the inflammatory activity in the classified clinical picture statements become possible for a differential indication of rheumatoid arthritis to a mud bath therapy in a health resort. The value of the therapy of the cures investigated is different for the various characteristic groups. The activity of inflammation changes in the course of the cure both in relation to the classification characteristics and in dependence upon the balneological basic diseases and the duration of the treatment. A list of indications is presented which shows parameters of effectiveness relevant to practice.
