Zusammenfassung
REBOA stellt eine temporäre Blutungskontrolle bei nicht komprimierbaren Körperstammblutungen des Abdomens, des Beckens oder bei stammnahen Amputationen der unteren Extremität dar. Dieses Verfahren soll in diesem Artikel genauer beleuchtet werden, da es in aktuellen Traumaleitlinien aufgeführt wird.
Abstract
REBOA is an old technique for temporary bleeding control of non-compressible torso haemorrhage in haemodynamic unstable patients not responding to standard therapeutic measures. The intraaortic balloon occlusion often result in a haemodynamic stabilisation and noticeable reduction in blood loss. Gaining early arterial (common femoral artery) access for potential REBOA patients is essential. REBOA occlusion time must be kept as short as possible to avoid ischaemic complications. Potential prehospital indication for REBOA are patients with non-compressible torso haemorrhage (massive blood loss due to pelvic fracture or intraabdominal bleeding) not responding to comprehensive standard therapeutic measures. Short prehospital length of time and prompt surgical bleeding control in the admitting hospital are essential in these patients. Literature data regarding prehospital REBOA are still limited but promising.
