Praxis der Lungensonografie in der Neonatologie – Teil 2

Die besondere Relevanz der Lungensonografie bei der Behandlung neonatologischer Patienten ergibt sich aus einer engen Korrelation zwischen dem Ausmaß sonografisch darstellbarer Pathologien und den Parametern einer respiratorischen Insuffizienz. Daraus lässt sich ein direkter Zusammenhang zwischen sonografischem Befund und der Schwere einer Lungenschädigung ableiten. Die Lungensonografie stellt somit eine einzigartige, ubiquitär verfügbare, bettseitige, seriell durchführbare Methode zur Überwachung des pulmonalen Status dar.

Kernaussagen
  • Die besondere Relevanz der Lungensonografie bei der Behandlung neonatologischer Patienten beruht auf der engen Korrelation zwischen dem Ausmaß sonografisch darstellbarer Pathologien und der respiratorischen Insuffizienz mit der Möglichkeit, einen direkten Zusammenhang zur Schwere einer Lungenschädigung abzuleiten.

  • Die Lungensonografie stellt eine einzigartige, ubiquitär verfügbare, bettseitige, seriell durchführbare Methode zur Überwachung des pulmonalen Status dar.

  • Bei Integration in die Stationsroutine als Point-of-Care-Verfahren kann durch den Lungenultraschall nicht nur eine deutliche Reduktion der kumulativen Strahlenexposition, sondern eventuell sogar auch eine Verbesserung der Behandlungsqualität erreicht werden.

  • Bei differenzialdiagnostischen Überlegungen müssen klinische Symptome, Laborbefunde und sonografisches Bild stets zusammen betrachtet werden.

  • Eine möglichst vollständige Erfassung der Lungenoberfläche verbessert vor allem im späteren Verlauf die diagnostische Sicherheit.
