Case Report: Successful Pre-hospital Red Blood Cell Transfusion in a Polytraumatized Patient Under Special Circumstances
Zusammenfassung
Polytraumatisierte Patienten nach Verkehrsunfällen zählen weltweit, vor allem aber in ländlichen Regionen aufgrund der längeren Transportwege, zu den kritischen Patientengruppen. Wir stellen einen Fall vor, bei dem die Stabilisierung und das Überleben eines jungen Menschen durch den Einsatz des Epoc-Systems (EPOC BGA-System, Siemens Healthineers) der DRF-Luftrettung und der Sonografie sowie einer unkonventionellen Entscheidung aller Beteiligten ermöglicht werden konnte.
Durch die Möglichkeit der Durchführung einer präklinischen Blutgasanalyse, sowie einer Point-of-Care-Sonografie (POCUS) wurde bei dem Patienten eine abdominelle Blutung diagnostiziert und die Indikation zur präklinischen Bluttransfusion gestellt. Der Patient konnte durch die Bereitstellung und Transfusion von Erythrozytenkonzentraten (EK) präklinisch stabilisiert werden.
Der Fall verdeutlicht, dass eine Vorhaltung von Blutprodukten auf geeigneten Rettungsmitteln ein verbessertes Outcome kritisch verletzter Patienten ermöglichen kann.
Abstract
Polytrauma patients following traffic accidents are among the most critical patient groups in the world, especially within rural regions due to the longer transport routes. We present a case of a young person who was stabilized and survived due to extended diagnostic possibilities of air rescue and an unconventional decision by all those involved.
The patient’s bleeding or hemorrhage was diagnosed using a preclinical blood gas analysis and a point-of-care sonography (POCUS). The bleeding was stabilized preclinically by providing and transfusion of erythrocyte concentrates.
The case illustrates that providing blood products on suitable rescue equipment can improve the outcome of critically injured patients.
