Abstract

Objective To investigate the influence of time spent viewing a video display terminal (VDTt), sleep, anxiety, and depression on the thickness of the lipid layer of the tear film (LLT).

Methods This is a cross-sectional study. A total of 102 patients were enrolled. Their ocular surface disease index (OSDI), Standard Patient Evaluation of Eye Dryness (SPEED) questionnaire, Pittsburgh Sleep Quality Index (PSQI), Hospital Anxiety and Depression Scale anxiety subscale (HADS-A), and depression subscale (HADS-D) were recorded. The LLT, meibomian gland secretion quality, tear film break-up time (TBUT), conjunctival hyperemia grade, corneal staining score, and VDTt were also evaluated. Spearman analysis was used to analyze the correlation. The patients were divided into the thinner LLT group (LLT < 75 nm) and the thicker LLT group (LLT ≥ 75 nm). The Mann-Whitney U test was used to analyze the characteristics and to compare the two groups. The receiver operating characteristic (ROC) curve was used to predict the relationship between VDTt and LLT.

Results Spearman correlation analysis showed a significant positive correlation between TBUT and average LLT, and significant negative correlations between mean LLT and OSDI, SPEED, meibomian gland secretion quality, conjunctival hyperemia, corneal staining, VDTt, PSQI, and HADS-A. In comparison to the thick LLT group, the thin LLT group had a worse OSDI score, shorter TBUT, longer VDTt, worse PSQI and HADS-A scores. The ROC curve indicated that the optimal cutoff value for using VDT to predict LLT less than 75 nm is 8.25 hours.

Conclusion The thinner LLT had worse questionnaire scores and signs related to dry eye, which indicates that a thinner LLT is more prone to causing dry eye. Using VDTt for more than 8.25 hours is more likely to induce thinning of the LLT, thereby increasing the risk of dry eye. Patients with a sleep disorder and anxiety are more susceptible to thinning of the LLT, suggesting that they are prone to developing dry eye syndrome.

Zusammenfassung

Zielsetzung Es sollte der Einfluss der Zeit, die mit dem Betrachten von Video-Display-Terminals (VDTt) verbracht wird, von Schlaf, Angst und Depression auf die Lipidschichtdicke des Tränenfilms (LLT) untersucht werden.

Methoden Unsere Studie ist eine Querschnittsstudie. Insgesamt wurden 102 Patienten in die Studie aufgenommen. Ihr Ocular Surface Disease Index (OSDI), der SPEED-Fragebogen (SPEED: Standard Patient Evaluation of Eye Dryness), der Pittsburgh Sleep Quality Index (PSQI), die Hospital Anxiety and Depression Scale Angst-Subskala (HADS-A) und die Depressions-Subskala (HADS-D) wurden erfasst. Außerdem wurden der LLT, die Qualität der Meibom-Drüsen-Sekretion, die Tränenfilmaufrisszeit (TBUT), der Grad der konjunktivalen Hyperämie, der Corneal Staining Score und der VDTt ausgewertet. Zur Analyse der Korrelation wurde die Spearman-Analyse verwendet. Die Patienten wurden in eine dünnere LLT-Gruppe (LLT < 75 nm) und eine dickere LLT-Gruppe (LLT ≥ 75 nm) eingeteilt. Der Mann-Whitney-U-Test wurde zur Analyse der Merkmale zwischen den beiden Gruppen verwendet. Die ROC-Kurve wurde verwendet, um die Beziehung zwischen VDTt und LLT vorherzusagen.

Ergebnisse Die Spearman-Korrelationsanalyse zeigte eine signifikante positive Korrelation zwischen TBUT und durchschnittlichem LLT, eine signifikante negative Korrelation zwischen durchschnittlichem LLT und OSDI, SPEED, Qualität der Meibom-Drüsen-Sekretion, konjunktivaler Hyperämie, Corneal Staining, VDTt, PSQI und HADS-A. Im Vergleich zur dicken LLT-Gruppe wies die dünne LLT-Gruppe einen schlechteren OSDI-Score, eine kürzere TBUT, eine längere VDTt, einen schlechteren PSQI- und HADS-A-Score auf. Die ROC-Kurve zeigte, dass der optimale Cut-off-Wert für die Verwendung der VDT zur Vorhersage von LLT unter 75 nm 8,25 h beträgt.

Schlussfolgerung Je dünner der LLT ist, desto schlechter sind die Fragebogenwerte und Anzeichen für ein trockenes Auge, was darauf hindeutet, dass ein dünner LLT eher zu einem trockenen Auge führen kann. Bei einer Nutzungsdauer von mehr als 8,25 h ist es wahrscheinlicher, dass der LLT dünner wird, wodurch sich das Risiko eines trockenen Auges erhöht. Patienten mit Schlafstörungen und Angstzuständen sind anfälliger für eine Ausdünnung des LLT, was darauf hindeutet, dass sie anfälliger für die Entwicklung eines Syndroms des trockenen Auges sind.
