Zusammenfassung
Die Kenntnis und gegebenenfalls Quantifizierung der Koronarmorphologie und der
linksventrikulären Funktion sind wichtige Voraussetzungen zur Entscheidungsfindung über
die adäquate Revaskularisationsmethode bei koronarer Mehrgefäßerkrankung. Sie müssen
jedoch unbedingt ergänzt werden durch eine Beurteilung der Morbidität der Patienten und
eine Besprechung im Herz-Team. Rahmenbedingungen können durch klinikinterne SOP,
insbesondere an Kliniken ohne eigene Herzchirurgie vor Ort, festgelegt werden. In die
letztendliche Entscheidung über die Wahl der Revaskularisationsmethode ist der Patient
einzubeziehen, am besten durch ein gemeinsames Gespräch, gegebenenfalls auch mit
Angehörigen.
Abstract
Knowledge and, if necessary, quantification of coronary morphology and left
ventricular function are important prerequisites for decision-making on the adequate
revascularization method in coronary multivessel disease. However, they must be
supplemented by an assessment of the morbidity of the patients and a discussion in the
heart team. Framework conditions can be defined by internal clinic SOPs, especially at
clinics without their own on-site cardiac surgery. The patient must be involved in the
final decision on the choice of the revascularization method, preferably through a joint
discussion, if applicable with relatives.
