Anamnese

Ein 35-jähriger Patient synkopiert beim raschen Überqueren der Kornhausbrücke in Bern, subjektiv auch in Verbindung mit einer gewissen Höhenangst. Passanten zufolge bestand für ca. 10 Sekunden ein Bewusstseinsverlust. Der Patient kommt anschließend zufällig an der Notfallaufnahme einer Berner Klinik vorbei und stellt sich mit persistierendem Unwohlsein, Palpitationen, „flachem Puls” und leichtem pektoralen Druckgefühl vor. Er gibt zudem schwache frontale Kopfschmerzen nach leichter Kopfkontusion, sonst jedoch keine Commotiosymptome an. Zurzeit bestehen vermehrter beruflicher Stress wegen einer zusätzlichen Weiterbildung und ein relativer Schlafmangel. Der Patient verneint es, Medikamente einzunehmen.

Kardiale Vorgeschichte

Rezidivierende Extrasystolen ohne Palpitationen, jedoch auffällig beim Pulsmessen, (ca. alle 4–5 Schläge 1 ES) sind seit dem 18. Lebensjahr bekannt, 2000 hätte er eine „Präsynkope” erlitten.

Früher habe er selten Dyspnoe bei starker Anstrengung und Orthostase bei tendenzieller Hypotonie entwickelt, zusätzlich seien noch ein Herzgeräusch oder ein atypischer Herzton bekannt. Dies sei aber im Rahmen der militärischen Tauglichkeitsuntersuchung gefunden und partiell abgeklärt worden; der Patient ist schließlich als militärdiensttauglich beurteilt worden.

Es sind keine Allergien oder medikamentöse Unverträglichkeiten bekannt, Noxen werden verneint (kein Nikotin, kein THC oder andere Drogen). Die Familienanamnese ist negativ bezüglich Kardiopathien, ein Onkel leidet an Diabetes mellitus.
