Elektromyo- und -neurografische Untersuchungen bei Sepsis- / SIRS-Patienten zur Verlaufsbeurteilung einer Critical-Illness-Myopathie und -Neuropathie (CRIMYN)

Zusammenfassung

Einführung Critical-Illness-Polyneuropathie (CIP) und -Myopathie (CIM) kommen bei Intensivpatienten häufig gemeinsam vor (CRIMYN). Beide führen zu Problemen der Entwöhnung vom Respirator. Die isolierte Betrachtung nur einer der beiden Entitäten ist unzureichend. In der vorliegenden Studie wurden durch frühzeitige sowie engmaschige elektrophysiologische Untersuchungen Beginn und Verlauf neuro- und myografischer Veränderungen erfasst. Methode Es wurden 30 Patienten mit Sepsis / SIRS (15 Frauen, Alter von 22–81 Jahre, Median: 69) prospektiv elektromyografisch und -neurografisch untersucht. Bei den Patienten wurden ab der 1. Woche nach Krankheitsbeginn auf der Intensivstation wöchentlich und nach einem halben Jahr die motorische Neurografie an 6 motorischen Nerven und die sensible Neurografie an 4 sensiblen Nerven durchgeführt. Aus den Messparametern wurden ein sensibler (SNCIx), ein motorischer (MNCIx) sowie ein gesamter Nervenleitindex (MSNCIx) berechnet. Die Elektromyografie erfolgte aus einem distalen und proximalen Muskel der oberen und unteren Extremitäten. Ergebnisse Im Median weisen alle 3 Scores (MSNCIx, MNCIx und SNCIx) auf eine pathologische Abnahme im Verlauf mit einer Normalisierung nach einem halben Jahr hin. 22 Patienten (22 / 30) hatten einen verminderten MSNCIx. Von den übrigen 8 Patienten verbesserten sich 4 Patienten im MSNCIx. Weitere 4 Patienten blieben unverändert, zeigten aber pathologische Spontanaktivität. Bei allen nachuntersuchten Patienten waren die pathologischen Veränderungen nach 6 Monaten komplett rückläufig. Schlussfolgerung Bei allen Patienten mit schwerer Sepsis ließ sich bereits innerhalb der ersten 7 Tage elektrophysiologisch eine neuromuskuläre Störung belegen. Nach einem halben Jahr bildete sich der CRIMYN vollständig zurück. Es konnte ein U-förmiger Schädigungsverlauf mit maximaler Läsion um die 3. bis 4. Krankheitswoche dargestellt werden.

Abstract

Introduction Critical illness polyneuropathy and myopathy (CIP / CIM) are frequently seen in critically ill patients with severe sepsis. CIP / CIM and subsequent muscular weakness are known to be associated with prolonged weaning from mechanical ventilation and prolonged stay in the ICU. This study was designed to describe the onset of electrophysiological signs of CIP / CIM and the prognosis in the process of a consequent systematic and close investigation. Methods 30 patients with sepsis / SIRS were studied prospectively. First electrophysiological investigation was assessed within 7 days, then every week and finally after 6 months. Motor and sensory conduction studies were performed from 6 motor and 4 sensory nerves as well as needle EMG from 4 muscles with assessment of spontaneous activity. Results The median motor nerve conduction index (MNCIx), sensory nerve conduction index (SNCIx) and motor and sensory nerve conduction index (MSNCIx) decreased over time and showed normal values after 6 months. In detail, 22 / 30 patients showed a decrease of the MSNCIx at the beginning. From the remaining 8 patients, 4 patients improved in the MSNCIx. 4 patients had no change during the ICU stay but showed pathological spontaneous activity in the EMG in the process. Conclusions We observed pathological signs in electrophysiological testing in all patients starting within the first 7 days after the onset of critical illness. Furthermore, all intensive care patients showed a U-shaped profile with a maximum lesion in the third and fourth week. In all survivors we saw a complete recovery after 6 months.
