Summary
Spinocranial neurofibromas are rare and like other tumors in this region often present diagnostic difficulties. In the presented case there was a composite neurological picture characterized by a Brown-Séquard syndrome with muscle atrophy and fasciculations in the left upper extremity, advanced tetraparesis with predominantly left motor and right sided sensory disturbances. There also was a remarkable atrophy with fasciculations at rest in the thigh muscles in the left lower extremity. Diagnosis was made after high cervical, basal cistern myelography which delineated the lower and upper margins of a spinocranial tumor.
At operation an anterolateral bilobed extra- and intradural neurofibroma protruding into the anterior foramen magnum, was found. The resection of the extra- and intradural parts of the tumor was a painstaking procedure. The spinal cord, which was compressed anteriorly and laterally, was grossly damaged, was yellowish-brown and had a waist-like pinched appearance.
The post-operative recovery was prolonged. Eventually the patient recovered from tetraplegia almost completely and today he is walking freely and is self-employed. He manufactures fine filigree metal jewellery necessitating a considerable amount of manual dexterity and precision.
Zusammenfassung
Cervikokraniale Neurofibrome sind selten und bieten, wie auch die anderen Tumore dieser Regionen oft diagnostische Schwierigkeiten. Im hier vorgestellten Fall war ein kombiniertes neurologisches Bild vorhanden mit einem Brown-Séquard-Syndrom und Faszikulationen im Bereich der linken oberen Extremität zusammen mit Tetraparese mit überwiegenden linksseitigen motorischen und rechtsseitigen sensiblen Ausfallserscheinungen. Außerdem war eine deutliche Atrophie mit Ruhefaszikulationen der Muskeln des linken Oberschenkels vorhanden. Die Diagnose wurde gestellt aufgrund einer hohen cervikalen und cisternalen Myelographie, die den oberen und unteren Tumorrand darstellt.
Bei der Operation wurde ein anterolaterales Zwerchsackneurinom mit extra- und intraduraler Lokalisation, das sich zum anterioren Anteil des Foramen magnum vorwölbte gefunden.
Die Resektion der extra- und intraduralen Anteile des Tumors wurde sorgfältig durchgeführt. Das Rückenmark, das vorn und lateral komprimiert wurde, war erheblich geschädigt, gelbbräunlich und taillenartig eingeschnürt. Die postoperative Erholung war verzögert. Der Patient erholte sich von seiner Tetraparese nahezu komplett; heute geht er frei umher und ist selbständig. Er fertigt Metallschmuckstücke an, die eine beträchtliche manuelle Fertigkeit und Präzision erfordern.

Les neurofibromes cervico-craniens sont rares et présentent souvent les difficultés de diagnostic communes aux autres tumeurs de cette région. Le cas présenté comportait un tableau neurologique rcomposite fait d'un syndrome de Brown-Séquard avec de l'atrophie musculaire et des fasciculations au membre supérieur gauche, une quadriparésie avancée avec prédominance de troubles moteurs à gauche et sensitifs à droite. On notait aussi une atrophie marquée avec fasciculation au repos des muscles de la cuisse gauche. Le diagnostic a été donné par une myélographie cervicale haute et une cysternographie qui délimitaient les bords supérieur et inférieur de la tumeur cervico-cranienne. L'intervention fit apparaître un neurofibrome antérolatéral bilobé intra- et extradural faisant saillie par le foramen magnum. La résection des parties intra- et extradurales exigea un travail pénible et minutieux. La moelle, grossièrement lésée par la compression, présentait une stricture de couleur jaune-brunâtre. La récupération post-opératoire a été longue. Le malade se remit cependant d'une façon complète de sa tétraparésie, se déplace aujourd'hui librement et travaille pour lui-même. Il fabrique des bijoux en filigrane métallique ce qui exige à un haut point dextérité et précision manuelles.
