Summary
A case of a 25-year-old woman is presented who suffered from a diffuse primary malignant lymphoma of the leptomeninges. Her initial symptoms were headache and visual impairment and she had marked papilledema. Arteriograms, brain scan and air studies showed no abnormalities and she was considered to have pseudotumor cerebri. There was marked improvement on corticoid therapy for about one year when the symptoms recurred and this time the increasing intracranial pressure was not responding to therapy, and she died with massive cerebral oedema and poor arterial perfusion of the brain. At autopsy only congestion and swelling were grossly visible on the brain but microscopic examination showed diffuse meningeal and some perivascular infiltration by malignant lymphoma. Lymph nodes, bone marrow and viscera were free of tumour. The nature and histogenesis of lymphomas of the central nervous system are discussed.

Es wird der Fall einer 25jährigen Frau beschrieben, die an einem primären malignen Lymphom der Leptomeningen litt. Ihre Anfangssymptome waren Kopfschmerzen und Sehverschlechterung; sie hatte ausgeprägte Stauungspapillen. Angiogramme, Hirnszintigramm und Pneumenzephalogramm ergaben keine Abnormalitäten. Es wurde eine Pseudotumor cerebri angenommen. Es stellte sich nach einer Kortikoid-Therapie eine erhebliche Besserung für etwa ein Jahr ein. Danach kehrte die Symptomatik wieder. Jetzt zeigte der erhöhte intrakranielle Druck keine Besserung auf die Therapie und die Patientin verstarb infolge des massiven Hirnödems und einer verminderten arteriellen Perfusion des Hirns. Bei der Sektion war makroskopisch lediglich eine Hirnschwellung sichtbar, aber die mikroskopische Untersuchung ergab eine diffuse meningeale und eine mäßige perivaskuläre Infiltration durch ein malignes Lymphom. Lymphknoten, Knochenmark und Viszera waren frei von Tumor. Die Art und Histogenese der Lymphome des Zentralnervensystems werden besprochen.

Présentation du cas d'une femme de 25 ans atteinte de lymphome primaire malin diffus des leptoméninges. Les premiers symptomes consistaient en céphalées, troubles visuels et oedème papillaire prononcé. Les artériographies, pneumoencéphalographies et scintillographie ne montraient pas d'anomalies si bien qu'on conclut à l'existence d'un syndrome pseudo-tumoral. Une thérapeutique aux corticoïdes détermina une nette amélioration pendant un an après quoi il y eut réapparation de la Symptomatologie et l'hypertension intracranienne ne fut plus influencée par le traitement. Un oedème cérébral prononcé et une insuffisance vasculaire encéphalique aboutirent au décès. A l'autopsie on ne constata macroscopiquement que l'oedème cérébral, mais l'examen microscopique montra une infiltration diffuse des méninges et des espaces périvasculaires par un lymphome malin. Les ganglions lymphatiques, la moelle osseuse et les viscères étaient épargnés. Discussion sur la nature et l'histogénèse du lymphome du système nerveux central.

Se presenta un caso de un linfoma primario maligno y difuso de leptomeninges en un hombre de 25 años. Las sintomas iniciales son dolor de abceza, pérdida visual y edema papilar. Los arteriogramas, estudios cerebrales y exploraciones con aire, no muestran anormalidades y se piensa que tiene un pseudotumor de cerebro. Hay un notable mejoramiento al establecer el tratamiento con corticoides durante un año, al cabo del cual los síntomas reaparecen elevándose la presión intracraneal que resiste al tratamiento, y fallece con edema cerebral masivo y disminuición de la irrigación arterial del cerebro. En la autopsia sólo se reconoce macroscópicamente congestión e hinchazón del cerebro pero el examen microscópico muestra infiltración difusa de las meninges y algunos vasos, por un linforma maligno. Los nódulos linfáticos, médula ósea y vísceras, están libres de tumor. Se discute la naturaleza e histogénesis de los linfomas del Sistema Nervioso Central.
