Prothetische Planung und Versorgung eines Patienten mit Unterkieferdefekt

Bei Patienten mit Unterkieferdefekten ist es oft unmöglich, gingivalgetragenen Zahnersatz einzugliedern. Auch durch die chirurgische Rekonstruktion entfernter Kieferabschnitte kann kein neuer Kieferkamm mit gleichwertiger Belastbarkeit geschaffen werden. Eine zufriedenstellende prothetische Versorgung dieser Patienten wird erst durch die Insertion von Implantaten erreicht. Es gibt unterschiedliche prothetische Versorgungskonzepte einer solchen Ausganssituation. Der vorliegende Patientenfall zeigt die prothetische Versorgung des Unterkiefers mit einem individuell gegossenen Kobalt–Chrom–Steg mit herausnehmbarer Prothese sowie die Rehabilitation des Oberkiefers mit Kronen und Brücken. Ausführlich wird die prothetische Herangehensweise von der Vorgeschichte, Planung, Vorbehandlung, Implantation bis hin zur prothetischen Versorgung dargestellt.

Complete gingival dentures can hardly be inserted to patients affected by mandible deficiency. Even after surgical reconstruction of the removed alveolar segments, it seems to be difficult to create a new alveolar ridge with an equal load–bearing capacity. Only the insertion of implants can give such patients a satisfying prosthetic restoration. Starting from this initial situation, there are different concepts of prosthetic restoration. The present case shows the prosthetic restoration of the mandible with an individual cast cobalt–chromium–bar with removable denture and the rehabilitation of the maxilla with crowns and bridges. This article describes in detail the prosthetic way of dealing with anamnesis, planning, pre–treatment, implantation and prosthetic restoration.
