Cyriax’ „Gesäßzeichen”[1]

Zusammenfassung

Hintergrund: In der Literatur werden wenige klinische Tests zur Erkennung schwerwiegender Pathologien im Beckenbereich erwähnt. Cyriax beschrieb bei der klinischen Untersuchung eine Triade von Untersuchungsbefunden und gab ihnen den Namen Sign of the buttock. Es wird der Fallbericht einer Patientin mit einem positiven Sign of the buttock dargestellt. Zusätzlich wurde die relevante Literatur untersucht, um weitere Fallberichte oder diagnostische Studien zu finden, die dieses Zeichen untersuchten.

Methode: Die subjektiven und objektiven Befunde einer Patientin mit einem positiven Sign of the buttock wurden in einem Fallbericht zusammengefasst. Nach dem Sturz von einem Pferd war das Röntgenbild der 25-jährigen Frau ohne Befund. Der Arzt vermutete eine Prellung und überwies sie zur Physiotherapie. Bei der klinischen Untersuchung wurde das Cyriax-Gesäßzeichen entdeckt. Eine darauffolgende Kernspintomografie zeigte eine Fraktur des Os sacrum und des Os pubis. Die Durchsuchung von Pubmed erfolgte anhand der Begriffe Cyriax‘s sign of the buttock und Noncapsular pattern and neoplasm.

Ergebnisse: Insgesamt wurden 3 relevante Studien identifiziert. 5 Patienten wiesen ein positives Sign of the buttock auf. Die jeweiligen Ursachen waren metastasierendes Adenokarzinom (2 Fälle), transzervikale Schenkelhals-, Femurschaft- und Azetabulumfraktur.

Schlussfolgerungen: Alle in der Literatur beschriebenen Fälle mit positivem Sign of the buttock wiesen die 3 typischen Komponenten dieses Zeichens auf: ein eingeschränkter gestreckter Beinhebetest (Lasèque), eingeschränkte Hüftflexion mit gebeugtem Knie und nicht kapsuläres Muster von Bewegungseinschränkungen an der Hüfte. Trotz der Tatsache, dass Cyriax‘ Beschreibung des Kapselmusters am Hüftgelenk fragwürdig ist, zeigten alle Patienten bei der Untersuchung ein Muster von Bewegungseinschränkungen, das nicht mit dem Kapselmuster von Cyriax übereinstimmt.

Abstract

Background: Few clinical tests are described in the literature for detection of serious pelvic region diseases. Cyriax described a triad of findings during the clinical examination and named it sign of the buttock. The case report of a patient with a positive sign of the buttock is portrayed. In addition, the relevant literature was reviewed to find further case reports or diagnostic studies examining this sign. Method: The subjective and objective findings of a patient with a positive sign of the buttock were summarised in a case report. After a horse accident a 25 years old woman had a negative X-ray. The physician suspected a contusion and referred her to physiotherapy. During clinical examination Cyriax’s sign of the buttock was detected. MRI investigation revealed sacrum and pubis fractures. The pubmed search was conducted by using the terms Cyriax’s sign of the buttock and noncapsular pattern and neoplasm.

Results: In total 3 relevant articles were identified. 5 patients showed a positive sign of the buttock. The actual causes were metastatic adenocarcinoma (2 cases), transcervical femur fracture, femural shaft fracture and acetabulum fracture.

Conclusions: All cases with positive sign of the buttock described in literature demonstrated the typical 3 components of this sign: limited straight leg raise test, restricted hip flexion with the flexed knee and noncapsular pattern of restricted hip motion. Despite the fact that Cyriax’s description of the hip joint capsular pattern is questionable, all patients showed a restricted movement pattern during hip examination that does not agree with Cyriax’s capsular pattern.
