Zusammenfassung

Vorhofflimmern (VHF) ist die am weitesten verbreitete Form der kardialen Arrhythmie. Von der Erkrankung sind insbesondere ältere Personen betroffen. Die Inzidenz des Vorhofflimmerns steigt mit zunehmendem Lebensalter auf über 15 % bei den über 90-Jährigen an. VHF ist ein Risikofaktor für systemische Embolien und ischämische Schlaganfälle, die zudem bei Auftreten mit schwereren neurologischen Beeinträchtigungen einhergehen. Neben einer Behandlung des VHF ist eine Antikoagulationstherapie erforderlich, die allerdings nur die Hälfte der Patienten im therapeutischen Zielbereich erhalten.

Die Kosten des VHF belaufen sich auf etwa 660 bis 924 Mio. Euro in Deutschland, wobei die Krankenhauskosten den größten Anteil verantworten. Die Kosten für die Behandlung von Schlaganfallpatienten mit VHF sind ca. 3000 Euro höher als die der Schlaganfall-Patienten ohne VHF.

VHF kann zu Beeinträchtigungen der persönlichen Befindens führen und führt insbesondere bei Patienten mit symptomatischem VHF zu einer schlechteren Lebensqualität.

Vor dem Hintergrund einer zunehmenden Alterung der Gesellschaft sollte versucht werden, das Versorgungsmanagement von Patienten mit VHF zu verbessern.

Abstract

Atrial fibrillation (AF) is the most frequent cardiac arrhythmia. AF affects especially the elderly. The incidence rises with increasing age to over 15 % for those over 90 years old. AF is a risk factor for systemic embolism and ischemic stroke. Stroke with AF is more severe with regard to neurologic impairment. In the management of AF an anticoagulation is essential. However, only half of the patients benefit from anticoagulation in therapeutic target ranges.

Costs associated with AF accrue to 660 to 924 Mio Euro in Germany with hospitalization being the major cost contributor. Management of stroke patients with AF is at 3000 Euro higher compared to those patients without AF.

AF has an impact on patients’ wellbeeing and impairs patients’ quality of life, particularly the symptomatic form.

With an increasing ageing of the society it is suggested to ensure that care management for patients with AF will be optimized.
