Die akute Pseudoobstruktion des Kolons (Ogilvie-Syndrom) – eine seltene Komplikation nach Ablatio mammae und Direktrekonstruktion durch freien TRAM-Flap

Zusammenfassung

Wir berichten über eine 62-jährige Patientin, welche eine Ablatio mammae mit Direktrekonstruktion mittels freiem mikrovaskulärem TRAM-Flap bei einem Angiosarkom der rechten Mamma erhielt. Postoperativ entwickelte sie ein Ogilvie-Syndrom und dessen Komplikationen mit Todesfolge. Bei pathophysiologisch multifaktorieller Genese des Ogilvie-Syndroms werden die aktuell führenden Theorien zur Genese sowie die Diagnostik und die entsprechenden Therapieoptionen anhand des vorliegenden Falles dargestellt und diskutiert.

Abstract

We report a case of a 62 year old woman who presented with Ogilvie’s syndrome as a complication of mastectomy with free TRAM flap reconstruction due to angiosarcoma of the right breast. In the acute postoperative period, the patient expired as a result of Ogilvie’s syndrome related complications. Several assumptions regarding the causes of Ogilvie’s Syndrome as well as current theories about aetiology, diagnosis and therapy are discussed in this case presentation.
