Two recent articles in UiM addressed second-look ultrasound (SL-US) after breast magnetic resonance imaging (MRI) [1]
[2]. You might be wondering about the usefulness of a second look. After all, isn't breast MRI the imaging method with the highest sensitivity for diagnosing breast cancer [3]? That is true, but the specificity of modern MRI (with dynamic examination after contrast administration) is significantly lower than the sensitivity of up to 100 % in numerous studies. As a result of the high sensitivity, "more" breast lesions are detected with breast MRI than with mammography and US. This includes lesions suspicious for malignancy and lesions classified as suspicious according to MRI-BIRADS that are only visible on MRI and not on mammography and real-time US. The German Radiological Society recently updated the recommendations for performing breast MRI [4]. Increasing numbers of breast MRI examinations and improved image quality result in an increase in the number of breast lesions that can only be detected with MRI [5].
A recent thorough overview [5] describes the diagnostic work-up that is necessary for these breast lesions. Re-assessment with mammography (...the "retrospectoscope", the re-assessment of image data with greater knowledge, in this case knowledge of a breast lesion on MRI, is an important instrument for the quality control of imaging methods...) and the implementation of targeted second-look US are the first and most important additional diagnostic steps here [5].
Most authors therefore understand SL-US of the breast as a targeted real-time US examination of a breast lesion detected on breast MRI but not be detected on mammography and ultrasound of the breast performed prior to MRI [6]. However, others understand SL-US as the targeted sonographic clarification of a suspicious lesion detected on breast MRI in general even if there is no prior mammography and/or US information [7]. Retrospective mammography evaluation and SL-US can be used to determine whether the MRI-suspicious lesion can be histologically clarified via mammography-guided or US-guided biopsy in a manner that is simple, cost-effective, and minimally stressful for the patient. Both techniques have numerous advantages compared to the significantly less available MRI-guided biopsy technique that is reserved for lesions that can only be detected on MRI but not on SL-US (or in a retrospective mammography evaluation) [5]
[6].
So how good is SL-US, how high is the detection rate, what must be taken into consideration, how is the MRI-suspicious lesion located, and how is the US morphology interpreted?
Ideally, SL-US should be performed by a single person with significant experience, knowledge and skill with respect to both breast US and breast MRI [5]. The MRI image data must be evaluated at the same time as the SL-US examination. "Searching" for a lesion described in an MRI report without image data is not productive. This is because not only the different image morphologies of lesions in MRI and US, but also in particular differences in lesion location between US and MRI must be taken into consideration. While the patient is in a prone position with the arms next to the body and the breast is subjected to no or only minimal compression in the dedicated breast coil during breast MRI, US of the breast is performed in a supine position or an inclined position often with the arms over the head. This results in significant differences in breast shape and can result in dramatic misinterpretation of the location of a lesion which affects the distance of the lesion from the breast wall (fascia), the distance of the lesion from the nipple, and the position of the lesion based on the clock [8]. Park et al. provide a very good overview of all aspects of SL-US [8]. It further complicates matters that the shape and size of lesions differ greatly between MRI and US as a result of the different patient positioning and US transducer compression. Many lesions are therefore measured smaller on US than on MRI [8]. The distance to other known lesions (e. g. cysts and fibroadenomas) also differs significantly between MRI and US. However, observation of "other" lesions is important for detecting the target lesion on US.
So how good is SL-US and how high is the detection rate?
The study published in UiM [2] provides a detailed BIRADS classification of lesions on MRI and US. 132 lesions in 101 patients were classified as MRI-BIRADS 0 (50.8 %), BIRADS 3 (25 %), and BIRADS 4 (23.5 %) [2]. There were 19 malignant and 113 benign lesions. The MRI-US correlation was successful in 78 of 132 lesions (59 %) but was different between malignant lesions (correlation in 95 %) and benign lesions (correlation in 53 %). 1 of 6 BIRADS 4 lesions on MRI that could not be detected with US was a carcinoma. The second study examined whether the US characteristics of breast lesions primarily found with US differ from lesions found with second-look US [1]. The answer is yes since second-look lesions are smaller (7 mm vs. 9 mm), more frequently have a blurred margin, and more frequently have an immediate retroareolar position or a position in the breast periphery compared to "first-look" detected breast lesions [1].
A recently published meta-analysis [6] summarizes the results of 17 included studies regarding SL-US in breast lesions. The detection rate has a broad range between 22.6 % and 82.1 % (pooled 57.5 %). Focal breast lesions ("mass lesions") had a higher detection rate than "non-mass lesions". It is very important that the detection is higher for malignant lesions than for benign lesions. However, it is even more important that the lack of detection of a lesion with SL-US does not make it possible to rule out a malignant lesion [6].
Therefore, the most important basic points regarding the clarification of suspicious breast lesions primarily detected only on MRI (BIRADS 4 and 5) are summarized [5]

More than 50 % of these lesions can be detected by SL-US.

Mass lesions are detected more frequently than non-mass lesions and malignant lesions are detected more frequently than benign lesions with SL-US.

The US-BIRADS classification of lesions does not affect the indication for histological clarification. This results from the MRI-BIRADS classification.

Lesions detected with SL-US should undergo US-guided biopsy.

It is obligatory to insert a metal clip after biopsy (for correlation with MRI control examinations in the case of a benign result).

Failure of SL-US to detect an MRI lesion does not rule out malignancy. Since the probability for malignancy of these lesions is > 2 % (>BIRADS 3), they should be histologically clarified by MRI-guided biopsy.
The US technique has brought significant improvements in recent years particularly in breast ultrasound. US detection of microcalcifications seems possible [9]
[10], new US criteria for the diagnosis of malignant breast lesions have been introduced [11], and elastography has become part of breast US diagnostics [12]

[16]. Can SL-US results be improved by these new techniques?
There is only minimal literature in this regard. In a prospective study regarding the use of shear wave elastography in SL-US (73 patients, 96 BIRADS 4 and 5 lesions on breast MRI), 1 of 29 lesions could only be detected with elastography and the technique was helpful in determining the exact location of 5 additional lesions [17]. An initial study shows that SL-US guided by real-time MRI navigation (the MRI data were acquired with the patient in a supine position) improves the detection of MRI breast lesions [18].
Therefore, second-look US following breast MRI is useful. 57 % of MRI lesions can be located by SL-US and can be histologically clarified by US-guided biopsy in a manner that is cost-effective and minimally stressful for the patient. It remains to be seen whether modern US techniques can improve the detection rate.
Zwei rezente Arbeiten in UiM beschäftigen sich mit Second-Look-Ultraschall (SL-US) nach Mamma-Magnetresonanztomografie (MRT) [1]
[2]. „Zweimal hinsehen“, oder „zum zweiten Mal hinsehen“, denken Sie vielleicht, wozu soll denn das gut sein? Und ist die Mamma-MRT nicht die Methode mit der höchsten Sensitivität zur Diagnose des Mammakarzinoms unter allen bildgebenden Verfahren [3]? Das schon, ist hier die Antwort, aber die Spezifität der modernen MRT (mit dynamischer Untersuchung nach Kontrastmittelgabe) ist deutlich geringer als die Sensitivität von bis zu 100 % in zahlreichen Studien. Die hohe Sensitivität der Mamma-MRT hat zur Folge, dass einfach „mehr“ Mammaläsionen als mit Mammografie und US erfasst werden. Darunter sind dann auch für Malignität suspekte und nach MRT-BIRADS als suspekt klassifizierte Läsionen, die nur in der MRT, nicht aber in Mammografie und Real-time-US sichtbar sind. Die Deutsche Röntgengesellschaft hat die Empfehlungen zur Durchführung der MRT der Mamma eben aktualisiert [4]. Steigende Untersuchungszahlen in der Mamma-MRT und verbesserte Bildqualität führen dazu, dass die Zahl ausschließlich MRT-detektierbarer Mammaläsionen zunehmen wird [5].
Eine rezente Übersicht [5] beschreibt sehr fundiert, welcher weiteren Abklärung diese Mammaläsionen zugeführt werden sollen. Dabei sind natürlich die neuerliche Beurteilung der Mammografie (…das „Retrospektoskop“, die neuerliche Beurteilung von Bilddaten auf einem höheren Wissensniveau, in diesem Fall Kenntnis einer Mammaläsion in der MRT, ist ein wichtiges Instrument der Qualitätskontrolle bildgebender Verfahren…..) und die Durchführung einer gezielten, neuerlichen Sonografie („second-look US“) die ersten und wichtigsten weiteren diagnostischen Schritte [5].
Die meisten Autoren verstehen daher unter einem SL-US der Mamma eine gezielte Real-time-US-Untersuchung einer in der Mamma-MRT erfassten Mammaläsion, die in der Mammografie und Sonografie der Mamma – vor der MRT – nicht detektiert werden konnte [6]. Andere verstehen darunter aber auch die gezielte sonografische Abklärung einer in der Mamma-MRT erfassten suspekten Läsion ganz allgemein, auch wenn es dazu keine vorangegangene Mammografie und/oder US-Information gibt [7]. Mithilfe der retrospektiven Beurteilung der Mammografie und des SL-US soll geklärt werden, ob die in der MRT suspekte Läsion nicht einfach, kostengünstig und für die Patientin auch wenig belastend durch Mammografie- oder US-geleitete Biopsie histologisch abgeklärt werden kann [5]. Beide Techniken haben zahlreiche Vorteile gegenüber der weit weniger verfügbaren MRT-geleiteten Biopsie, die dann solchen Läsionen vorbehalten bleibt, die ausschließlich in der MRT, nicht aber im SL-US (und in der retrospektiven Beurteilung der Mammografie) erfasst werden können [5]
[6].
Wie gut ist nun der SL-US, wie hoch ist die Detektionsrate, was gilt es dabei zu beachten, wie finden wir die MR-suspekte Läsion und wie interpretieren wir die US-Morphologie?
Im Idealfall sollte der SL-US von einer Person durchgeführt werden, die hohe Erfahrung, Kenntnisse und Fertigkeiten sowohl mit Mamma-US als auch mit der MRT der Mamma hat [5]. Die Beurteilung der MRT-Bilddaten zeitgleich mit der SL-US-Untersuchung ist selbstverständlich, das „Suchen“ einer in einem MRT-Befund beschriebenen Läsion ohne Bilddaten wenig zielführend. Denn: Zu beachten sind nicht nur die differente Bildmorphologie durch unterschiedliche zugrundeliegende Technik, sondern ganz besonders auch Unterschiede der Lokalisation der Läsion zwischen US und MRT. Während bei der MRT der Mamma die Patientin in Bauchlage positioniert ist, die Arme am Körper angelegt sind und die Mamma keine oder nur geringe Kompression in der dedizierten Mammaspule ausgesetzt ist, erfolgt die US-Untersuchung in Rücken- oder Schräglage, oft mit den Armen über dem Kopf. Das führt zu deutlichen Unterschieden der Mammaform und kann zu dramatischen Fehleinschätzungen der Lokalisation einer Läsion führen, was Distanz zur Brustwand (Faszie), Abstand der Läsion zur Mamille und Position der Läsion im Hinblick auf die Uhr betrifft [8]. Eine sehr gute Übersicht zu allen Aspekten des SL-US finden sie bei Park et al. [8]. Dazu kommt erschwerend, dass sich auch die Form und Größe der Läsionen durch unterschiedliche Patientenlagerung, aber auch durch die Kompression mit dem Schallkopf zwischen MR und US deutlich unterscheiden. Viele Läsionen werden in der Sonografie daher kleiner gemessen als in der MRT [8]. Und auch der Abstand zu anderen, bekannten Läsionen (wie z. B. zu Zysten oder Fibroadenomen) ist dann zwischen MRT und US deutlich different. Trotzdem ist die Beachtung „anderer“ Läsionen eine wichtige Hilfe, um die Zielläsion sonografisch zu detektieren.
Wie gut ist nun der SL-US, wie hoch ist die Detektionsrate?
In der in UiM publizierten Studie [2] wird eine detaillierte BIRADS-Klassifikation der Läsionen in MRT und US angegeben. 132 Läsionen bei 101 Patientinnen wurden in MRT-BIRADS 0 (50,8 %), BIRADS 3 (25 %) und BIRADS 4 (23,5 %) klassifiziert [2]. Es fanden sich 19 maligne und 113 benigne Läsionen, die Korrelation MRT-US gelang bei 78 der 132 Läsionen (59 %), war allerdings bei malignen Läsionen (Korrelation 95 %) und benignen Läsionen (Korrelation 53 %) unterschiedlich. Bei einer von 6 BIRADS 4-Läsionen in der MRT, die nicht mit US detektiert werden konnte, handelte es sich um ein Karzinom. Die zweite Arbeit ging der Frage nach, ob die US-Charakteristika zwischen Mammaläsionen, die primär mit US gefunden werden, sich von second-look-gefundenen Läsionen unterscheiden [1]. Das tun sie, denn Second-look-Läsionen sind kleiner (7 mm vs. 9 mm), zeigen häufiger eine unscharfe Begrenzung und liegen häufiger unmittelbar retroareolär oder in der Mammaperipherie als „First-look“-Mammaläsionen [1].
Eine rezent publizierte Metaanalyse [6] fasst die Ergebnisse von 17 inkludierten Studien zum SL-US bei Mammaläsionen zusammen. Die Detektionsrate zeigt eine große Streubreite zwischen 22,6 und 82,1 % (gepoolt 57,5 %). Dabei hatten fokale Mammaläsionen („mass lesions“) eine höhere Detektionsrate als „non-mass-lesions“. Und was ganz wichtig ist: Die Detektion ist für maligne Läsionen höher als für benigne Läsionen. Noch wichtiger ist allerdings, dass die fehlende Detektion einer Läsion mit SL-US eine maligne Läsion nicht ausschließen lässt [6].
Daher sind die wichtigsten Eckpunkte zur Abklärung primär ausschließlich in der MRT detektierter suspekter Mammaläsionen (BIRADS 4 und 5) zusammenzufassen [5]

Das setzen eines Metallclip nach Biopsie ist obligat (zur Korrelation mit MRT-Kontrolluntersuchungen bei benignem Ergebnis).

Eine fehlende Detektion einer MRT-Läsion mit SL-US kann Malignität nicht ausschließen. Da die Wahrscheinlichkeit für Malignität dieser Läsionen > 2 % ist (>BIRADS 3), sollten sie durch MR-geleitete Biopsie histologisch abgeklärt werden.
Die US-Technik hat in den vergangenen Jahren gerade in der Mammasonografie massive Verbesserungen gebracht. Die US-Detektion von Mikrokalk scheint möglich [9]
[10], neue US-Kriterien zur Diagnose maligner Mammaläsionen wurden vorgestellt [11], und die Elastografie hat in der Mammadiagnostik Einzug gehalten [12]

[16]. Kann durch neue Techniken das Ergebnis mit SL-US verbessert werden?
Dazu gibt es wenig Literatur. In einer prospektiven Studie zum Einsatz der Scherwellenelastografie bei SL-US (73 Patientinnen, 96 BIRADS 4 und 5 Läsionen in der Mamma-MRT) konnte eine von 29 malignen Läsionen nur mit der Elastografie detektiert werden, bei weiteren 5 Läsionen war die Elastografie zur exakten Lokalisation hilfreich [17]. Und eine erste Studie informiert, dass der durch MR-Real-time-Navigation (die MR-Daten wurden in Rückenlage der Patientin akquiriert) geleitete SL-US die Detektion von MRT-Mammaläsionen verbessert [18].
„Noch einmal hinsehen“, mit US, nach Mamma-MRT, ist daher sinnvoll. 57 % der MRT-Läsionen können durch SL-US gefunden und durch US-geleitete Biopsie histologisch kostengünstig und wenig belastend für die Patientin abgeklärt werden. Ob moderne US-Techniken die Detektionsrate verbessern können, bleibt abzuwarten.
