Operative Versorgungsmöglichkeiten des diabetischen Fußsyndroms mit komplexer Problematik

Zusammenfassung

Das diabetische Fußsyndrom (DFS) umfasst als Überbegriff eine Vielzahl heterogener extremitätenbedrohender Krankheitsbilder, die auch beim Nicht-Diabetiker auftreten können. Für die orthopädische Betrachtung stehen folgende Problemkreise im Vordergrund: Durchblutungsstörungen (arterielle Verschlusskrankheit), Wunden und Weichteilinfektionen sowie Charcot-Fuß (diabetisch-neuropathische Osteoarthropathie) und Osteomyelitis. Der Artikel benennt behandlungsrelevante Aspekte dieser Erkrankungen, beschreibt gängige antibiotische und chirurgische Therapien und weist auf alternative, wo möglich, amputationenvermeidende Verfahren hin.

Summary

The diabetic foot syndrome as an umbrella term includes numerous heterogeneous extremity threatening disease patterns, which not only occur in diabetics. From an orthopaedic point of view the following problems have priority: circulation disorders (arterial occlusion disease), wound and soft tissue infections, Charcot foot (diabetic neuropathic osteoarthropathy) and osteomyelitis. The article lists treatment relevant aspects of these diseases, describes common antibiotic and surgical therapies and refers to alternative and, where possible, amputation avoiding procedures.
