Disseminated intravascular coagulation in meningococcal sepsis

Summary
We report on a man (age: 49 years), who died from severe meningococcal sepsis with disseminated intravascular coagulation (DIC), multiple organ dysfunction syndrome and extended skin necrosis. We discuss in detail the pathophysiology of the activation of coagulation and fibrinolysis during sepsis. The article discusses new therapeutic concepts in the treatment of disseminated intravascular coagulation in meningococcal sepsis, too.
Zusammenfassung
Wir berichten über einen 49-jährigen Patienten, der an einer schweren Meningokokkensepsis mit disseminierter intravasaler Gerinnung, schwerem Organversagen und ausgedehnten Hautnekrosen verstarb. Wir diskutieren detailliert die Pathophysiologie der Gerinnungs- und Fibrinolyseaktivierung während der Sepsis. Außerdem werden mögliche Ursachen besprochen, die zum Überwiegen der Gerinnungsaktivierung in dieser klinischen Situation und schließlich zur Fibrinablagerung in der Mikrozirkulation führen. Neue Behandlungskonzepte der disseminierten intravasalen Gerinnung bei Meningokokkensepsis werden diskutiert.
