Summary

The degree of permeability of the alveolar-capillary barrier to low molecular weight solutes has been supposed to depend upon the radius of the epithelial pori. The bullous emphysema is a natural model in which this radius may increase because of the high tension in the bulla. Accordingly, the 99mTc-DTPA clearance may be expected to be faster than normal in lung fields including bullae. Results of the present study in which 99mTc- DTPA clearance was measured in six patients with bullous emphysema, seem to confirm this hypothesis at least to some extent. In fact, either increased or normal 99mTc-DTPA clearances were found over the lung fields including bullae communicating with the bronchial tree. Differences in both the size and the inner tension of the bullae as well as in the anatomical and functional conditions of the surrounding parenchyma are likely to account for such varying behavior.

Zusammenfassung

Man vermutet, daß bei Verwendung von Lösungen mit niedrigem Molekulargewicht der Grad der Permeabilität der alveolo-kapillaren Barriere vom Radius der epithelialen Poren abhängt. Das bullöse Emphysem Modell, ist wahrscheinlich ein natürliche in welchem sich der Radius durch die Spannung in der Bulla erhöht. Folglich ist anzunehmen, daß die 99mTc-DTPA-Clearance in den Lungenfeldern, die Bullae enthalten, schneller erfolgt als normal. Die Ergebnisse der vorliegenden Studie, bei der die 99mTc-DTPA-Clearance bei 6 Patienten mit bullösem Emphysem gemessen wurde, scheinen diese Hypothese wenigstens teilweise zu bestätigen. Man fand tatsächlich eine erhöhte oder normale 99mTc-DTPAClearance in Lungenfeldern mit Bullae, die in Verbindung mit dem Bronchialbaum standen. Dieses unterschiedliche Verhalten dürfte durch die Unterschiede in Größe und innerer Spannung der Bullae, wie auch durch die anatomischen und funktionellen Zustände des umgebenden Parenchyms erklärbar sein.
