Abstract

Le diagnostique d’une hypertension rénale a été établi par la néphrographie isotopique, l’urographie, l’artériographie rénal et l’examen histologique du matériel de biopsie chez un malade de 36 ans. Après une chirurgie de restitution de l’artère rénale gauche la pression sanguine s’est abbaissée à des valeurs normales et l’état clinique du malade s’est amélioré. Avant l’opération, un „wash-out“ par le Furosemide n’a pu être obtenu que sur le côté sain tandis que le côté malade a conservé une opacification élevée du moyen de contraste. Après l’opération, on a pu observer un „wash-out” complet des deux côtés. Une modification distincte de la courbe néphrographique du rein gauche confirmait le succès de l’opération restitutrice. Le pyélogramme „wash-out” par le Furosémide et le rénogramme par le Furosémide sont recommandés comme tests screening additionels pour le diagnostique de l’hypertension rénale.

Summary

In a 36-years old man renovascular hypertension was diagnosed on the basis of positive findings obtained by radioisotope renography, rapid-sequence excretory urography, renal arteriography and histologic examination of biopsy material. After reconstructive surgery of the left renal artery the arterial blood pressure became normal and the clinical condition of the patient improved. Before operation a wash-out phenomenon could be induced by furosemide on the healthy side only whereas increased density remained on the affected side. After surgery a complete bilateral wash-out could be seen. A marked change in the configuration of the renographic curve of the left kidney during furosemide diuresis confirmed the success of the reconstructive procedure. The furosemide wash-out pyelogram and the furosemide renogram are recommended as further screening tests for renovascular hypertension.

Bei einem 36jährigen Mann wurde eine renale Hypertonie mittels der Isotopennephrographie, der Urographie, der renalen Artériographie und der histologischen Untersuchung von Biopsiematerial diagnostiziert. Nach Wiederherstellungschirurgie der linken Nierenarterie sank der Blutdruck auf normale Werte ab und der klinische Zustand des Patienten besserte sich. Vor der Operation konnte ein Wash-out durch Furosemid nur auf der gesunden Seite bewirkt werden, während auf der erkrankten Seite erhöhter Kontrastmittelschatten zurückblieb. Nach der Operation konnte ein kompletter Wash-out auf beiden Seiten beobachtet werden. Eine deutliche Veränderung der nephrographischen Kurve der linken Niere bestätigte den Erfolg der Wiederherstellungsoperation. Das Furosemid Wash-out Pyelogramm und das Furosemid Renogramm werden als weitere Screeningteste zur Diagnose der renalen Hypertonie empfohlen.
