Les avantages et les désavantages du 99m Tc O4 , qui n’est accumulé que passagèrement dans la glande thyroïdienne, sont comparés
â ceux du 131 I qui participe physiologiquement à la fonction de la thyroïde. En tout, 67 malades
ont été examinés et les scintigrammes de la thyroïde obtenus avec les deux radionuclides
ont été comparés quantitivement et qualitativement et mis en rapport avec les données
cliniques.
Le poids thyroïdien, basé sur la scintigraphic du 99m TcO4 , montre un taux de défaut plus élevé que celui basé sur la scintigraphic à l’iode
131. Ce défaut est causé probablement par le bruit de fond plus élevé du 99m TcO4 . Qualitativement les scintigrammes gagnés des cas euthyroïdiens et des thyreotoxicoses
sont en correspondances avec les données cliniques. Un manque de correspondance est
constaté dans les goitres nodulaires euthyroïdiens. L’application du 99m TcO4 devrait être limité l’évaluation des hyperthyréoïdes. Pour l’estimation du poids
de la thyroïde le scintigramme de l’iode doit être utilisé. Les résultats montrent
que pour l’évaluation des conditions euthyreoïdes du goitre nodulaire un isotope de
l’iode est préférable. Le remplacement de l’iode par le 99m TcO4 dans la scintigraphic de la thyroïde peut amener à une perte d’information surtout
dans le goitre nodulaire.
Summary
The advantages and disadvantage of the radiopharmaceutical 99m TcO4 , which has only a transient sojourn in the thyroid, were compared with those of another
radiopharmaceutical, 1-131, which is physiologically involved in thyroid function.
A total of 67 patients were studied. Thyroid scans obtained using both radionuclides
were compared quantitatively and qualitatively and correlated with the clinical status.
The weight calculated on the basis of the 99m TcO4 scan had a higher percentage of error than that calculated on the basis of the
1-131 scan. The error in weight is probably contributed to by the high background
radiation associated with 99mTc04. Qualitatively, the scans obtained with both radionuclides
are concordant with the clinical status in the case of euthyroid normal (control)
and hyperthyroid Graves' disease. Discordant results may be obtained in euthyroid
multinodular goiter. The use of 99m TcO4 should probably be restricted to the evaluation of hyperthyroid clinical status.
An iodine scan should be used for thyroid weight calculation. Results also indicate
that an isotope of iodine should be used for the evaluation of euthyroid clinical
status associated with multinodular goiter. Replacing an isotope of iodine with an
agent like 99m TcO4 for thyroid scans may result in a loss of useful clinical information, and in
some cases may contribute to false impression, especially in the case of multinodular
goiter.
Die Vorteile und Nachteile des 99m TcO4 , das nur vorübergehend in der Schilddrüse verbleibt, wurden mit jenen des 131 J verglichen, das physiologisch an der Funktion der Schilddrüse beteiligt ist. Insgesamt
wurden 67 Patienten untersucht, wobei Szintigramme der Schilddrüse, die mit beiden
Radionukliden erhalten wurden, quantitativ und qualitativ verglichen und auf den klinischen
Zustand bezogen wurden.
Das auf der Grundlage des 99m TcO4 -Szintigramms errechnete Schilddrüsengewicht zeigte eine höhere Fehlerquote als jenes,
das aus dem 131 J-Szintigramm berechnet wurde. Dieser Fehler ist wahrscheinlich auf die hohe Untergrundstrahlung,
die mit 99m TcO4 verbunden ist, zurückzuführen. Qualitativ stimmen die Szintigramme, die mit beiden
Radionukliden erhalten wurden, bei normalen euthyreoten Fällen und bei Thyreotoxikosen
mit dem klinischen Bild überein. Fehlende Ubereinstimmung kann bei euthyreoten Knotenkröpfen
beobachtet werden. Die Verwendung von 99m TcO4 sollte wahrscheinlich auf die Auswertung der Hyperthyreosen beschränkt werden. Zur
Berechnung des Schilddrüsengewichtes sollte das Jodszintigramm verwendet werden. Die
Ergebnisse weisen ferner darauf hin, daß für die Auswertung des euthyreoten Zustandes
bei einem Knotenkropf ein Jodisotop verwendet werden sollte. Der Ersatz eines Jodisotops
durch eine Verbindung wie 99m TcO4 in der Schilddrüsenszintigraphie kann, besonders beim Knotenkropf, zum Verlust klinischer
Information führen.
