Zusammenfassung

Das idiopathische Parkinsonsyndrom ist eine progrediente Erkrankung, deren motorischen Symptome in den ersten Jahren gut therapiert werden können. Nach einigen Jahren treten motorische Komplikationen auf, die eine differenzierte Therapie und den Einsatz verschiedener Substanzen erfordern. Dabei können zwar Empfehlungen ausgesprochen werden, eine individualisierte Einstellung ist jedoch unabdingbar. Neben den motorischen Symptomen nehmen im Verlauf die nicht motorischen Störungen zu, wobei besonders psychiatrische und autonome Symptome zu nennen sind. Erschwert wird die medikamentöse Therapie durch pharmakologische Interaktionen sowie die häufig bestehende Multimorbidität.

Summary

Parkinson’s disease is a chronic progressive disorder, the motor symptoms of which can be well controlled by treatment during the first years. After a number of years, motor complications are likely to occur that require differentiated treatment and the use of various therapeutic measures. Although recommendations may be made, individualized management will be indispensable. Aside from motor symptoms, non motor disturbances are aggravated in the course – psychiatric and autonomic symptoms in particular. Drug treatment is often tangled by pharmacologic interactions and concomitantly existing multimorbidity.
