Nine maxillary sinuses of 8 patients who had undergone Caldwell-Luc surgery (CWL)
were injected with 100-125 µCi 99mTc-DTPA in 0.2 ml from the gingivobuccal sulcus, passing through the canine fossa
window. Sinus half-emptying times and percent contribution of the natural ostium (NO)
and the nasoantral window (NA) to the clearance from the sinus were calculated, using
time-activity curves as well as the activity of cotton wool plugs placed into the
middle and lower conchae. Mean half-emptying times in patients in whom the mucosa
had been removed were shorter than those in patients in whom it had been left in place.
In the former group percent drainage through the NO was higher than that through the
NA whereas in the latter group the opposite was the case. These findings are consistent
with the thesis that if the mucosa is retained, the ciliae continue their sweeping
action towards the NO but if it is removed the NA gains importance in the drainage.
This method is promising for the evaluation of maxillary sinus drainage following
CWL.
Zusammenfassung
Bei 8 Patienten mit chronischer Oberkieferhöhlenentzündung, die nach Caldwell-Luc
(CWL) operiert worden waren, wurden 100-125 µCi 99mTc-DTPA in 0,2 ml vom gingivo- bukkalen Sulcus aus in den Sinus injiziert. Danach
wurden die Entleerungs-Halbzeiten aus dem Sinus und die prozentualen Anteile des natürlichen
Ostiums (NO) und des nasoantralen Fensters (NA) an der Clearance berechnet, wobei
sowohl die Zeit/Aktivitäts-Kurven als auch die Aktivität von Tupfern herangezogen
wurden, die in die mittlere bzw. untere Concha gelegt worden waren. Bei Patienten,
bei denen die Mukosa entfernt worden war, waren die durchschnittlichen Entleerungszeiten
kürzer als bei Patienten, bei denen sie belassen worden war. Bei der ersteren Gruppe
war der Drainage-Anteil durch das NO höher als jener durch das NA, während bei der
letzteren Gruppe das Gegenteil der Fall war. Diese Beobachtungen stützen die These,
wonach die Belassung der Schleimhaut den Zilien ihre Transportfähigkeit in Richtung
NO erhält, während nach ihrer Entfernung das NA an Bedeutung für die Drainage gewinnt.
Die Methode scheint für die Bewertung der Drainage des Sinus maxillaris nach CWL vielversprechend
zu sein.
