Zusammenfassung

Auf dem Gebiet der therapieresistenten Depressionen fanden in den letzten Jahren wesentliche Entwicklungen statt, die nicht nur die Definition und Klassifikation betreffen, sondern auch unsere Kenntnisse über Ursachen und Determinanten der Therapieresistenz. Gleichzeitig wurden zahlreiche neue Behandlungsverfahren eingeführt, deren Stellenwert in einem sinnvollen, evidenzbasierten Therapiekonzept kritisch zu überprüfen ist. Der vorliegende Beitrag gibt einen Überblick über den gegenwärtigen Wissensstand und zeigt Forschungsperspektiven auf, die sowohl eine Erweiterung unserer Kenntnisse über Ätiologie und Pathogenese depressiver Erkrankungen als auch die Entwicklung innovativer Behandlungsansätze erwarten lassen.

Summary

In recent years, developments in the field of treatment-resistant depression not only brought about improved definition and classification, but also led to an increase of our knowledge on the etiology and determinants of treatment resistance. A number of novel treatment strategies have been suggested which will have to be critically evaluated and integrated into an evidence-based therapeutic concept. The present contribution provides an overview of these developments and reveals promising research perspectives which can be expected to bring about an extension of our knowledge on the etiology and pathogenesis of depressive disorders and may thereby lead to the development of innovative therapeutic strategies for treatment-resistant depression.
