Abstract

We have designated the validity of a diagnostic test by a single function S taking into account the values of clinical specificity, sensitivity, and the morbidity rate. Thus, by the use of this function each possible diagnostic test can be characterized and easily differentiated from each other.

When integrating function S over all x’s (morbidity rates), a parameter I_s is obtained which can provide information on the total diagnostic capacity of a test because it comprises the diagnostic properties of both the positive and the negative test results (p₊ and p_-) in all possible samples with the morbidity rate (x) ranging from 0 to 1.

p₊ and p_- parameters being a posteriori probabilities were used to denote the diagnostic validity,of the positive and the negative test results respectively. By these parameters diagnostic conclusions could be drawn even from unselected population samples (with x values between 0 and 1) in which sensitivity and specificity parameters have no diagnostic properties.

Wir haben die Trennschärfe eines diagnostischen Tests durch eine einzige Funktion S, in der die Werte für klinische Spezifität, Empfindlichkeit und die Morbiditätsrate berücksichtigt werden, ausgedrückt. Mit Hilfe dieser Funktion kann jeder diagnostische Test charakterisiert und von anderen Tests unterschieden werden.

Wenn man die Funktion S über alle Morbiditätswerte integriert, erhält man einen Parameter I_s, der über die gesamte diagnostische Kapazität eines Tests informiert, da er die diagnostischen Eigenschaften der positiven und negativen Testergebnisse (P₊ und P_-) in allen Stichproben mit der Mortalität von 0 bis 1 erfaßt.

P₊ und P_- wurden als A-posteriori-Wahrsche’.nlichkeiten benutzt, um die diagnostische Validität der positiven und negativen Testergebnisse zu bezeichnen. Mittels dieser Parameter konnten diagnostische Schlußfolgerungen selbst aus unausgelesenen Bevölkerungsstichproben (mit Morbiditätsraten zwischen 0 und 1), in denen den Parametern Empfindlichkeit und Spezifität keine diagnostische Bedeutung zukam, gezogen werden.
