Komplikationen in der orthopädischen Kinderrheumatologie

Zusammenfassung

Operationen im Kindes- und Jugendalter bei rheumatischen Erkrankungen stehen im Gegensatz zu der Erwachsenenrheumatologie am Ende der Therapieoptionen [1]. Über die Hälfte der betroffenen Patienten leiden an einer Monarthritis oder Oligoarthritis [2]. Bei inkomplettem Ansprechen auf die medikamentöse Basistherapie entbrennt ein Wettlauf um ein gelenkerhaltendes Vorgehen und bei entzündlich-destruktiv veränderten Gelenken die Frage der adäquaten konservativen oder operativen Vorgehensweise. Möglichkeiten und Grenzen einer operativen Therapie bei jugendlichen Rheumatikern sollen im Folgenden dargestellt werden und die Komplikationen dabei bedacht werden.

Abstract

For children and adolescents with rheumatological diseases, surgery is a last-line treatment option [1] in contrast to rheumatology treatment in adults. More than half of affected patients suffer from mono- or oligoarthritis [2]. In cases of insufficient response to basic drug treatment, the focus is on joint-preserving procedures, and joints with inflammatory destructive changes raise the question of the most adequate conservative or surgical approach. This article aims to present chances and limits of surgical treatment in young rheumatic patients while giving due consideration to complications.
