Zusammenfassung
Die Syringozele als zystische Erweiterung der Ausführungsgänge der Glandulae bulbourethrales
Cowperi wird in der Literatur nur selten und dann überwiegend bei Kindern beschrieben.
Wir berichten über den Fall einer späten klinischen Manifestation der bulbären Form
einer geschlossenen Syringozele im Erwachsenenalter, die als perineale zystische Raumforderung
imponierte. Die Diagnostik beruht auf Sonographie, Urethrographie und Urethrozystoskopie.
Neben der Sonographie erwies sich in unserem Fall die Kernspintomographie als exzellentes
bildgebendes Verfahren zur Darstellung der Lagebeziehung der Syringozele zu Urethra
und Corpus spongiosum. Therapeutisch erfolgte die offene Revision über einen perinealen
Zugang. Bei unklaren Beschwerden im Perinealbereich sollte auch beim Erwachsenen an
das Vorliegen einer Syringozele als mögliche Ursache gedacht und eine entsprechende
Diagnostik eingeleitet werden.
Abstract
Cystic dilatation of bulbourethral gland ducts is a rare pathology known as Cowper's
syrincocele. It is normally found in children and young adults. We report a case of
late clinical manifestation of a large dilatation of Cowper's gland duct presenting
as a perineal cyst. Diagnostic procedures consisted of ultrasonography, urethrography
and urethrocystoscopy. Magnetic resonance imaging proved to be an excellent tool to
demonstrate the topographic relationship between the syringocele and the urethra and
corpus spongiosum. The syringocele was treated successfully by open perineal excision.
As the syringocele seems to be more common than previously reported, the physician
should consider it in patients with unusual genitourinary symptoms.
