Abstract

Zusammenfassung.

Bericht über einen tödlichen Zwischenfall während der Vojta-Therapie bei einem 3 Monate alten weiblichen Säugling durch Einblutungen in die Gefäßscheiden beider Vertebralarterien in Höhe ihrer Atlasschlingen bei C1. Der Tod des Kindes erfolgte durch Minderperfusion des kaudalen Hirnstamms mit begleitender Subarachnoidalblutung bei starker aktiver Kopfdrehung und Reklination und ereignete sich bereits während der 1. therapeutischen Sitzung. Neben dem klinischen Ablauf werden die neuropathologischen Details dargestellt. Es ist zu vermuten, dass ähnliche Verläufe sich bereits früher ereigneten, ohne dass darüber berichtet wurde. Symptome einer Hirnstammschädigung können hierbei auch um Stunden, seltener Tage, zeitverzögert einsetzen. Die Risiken einer Manualtherapie beim Kind werden anhand dieses Falles diskutiert.

Detailed clinical and neuropathological report on a fatal incident during the first manual therapy according to Vojta conducted in a 3 months old baby: during forced active rotation and head retraction the baby suffered from a bleeding into the adventitia of both her vertebral arteries at the level of C1 prompting ischemia of the caudal brainstem with subarachnoid haemorrhage around. It has to be suggested that similar cases already have occurred but have not been reported yet. There might be a time lag between the performance of physiotherapy and the beginning of neurologic symptoms. The risks of manual therapy in children will be discussed.
