Zusammenfassung
21 Personen mit rechtshirnigem Mediainfarkt wurden entweder mit Imipramin (n = 11) oder mit Piracetam (n = 10) behandelt und mit zwölf Kontrollpatienten (gleiche Ätiologie) verglichen. Messparameter waren die Leistung in der visuo-räumlichen Aufmerksamkeit (Neglekttest und visuelles Scanning aus der Testbatterie zur Aufmerksamkeitsprüfung), die Alertness, das funktionelle Outcome (Functional independence measure, FIM) und das subjektive Befinden vor und nach einer 4-wöchigen Behandlung. Alter und Intelligenz wurden zwischen den Gruppen gematcht. Die Untersuchung fand im Rahmen einer stationären Rehabilitationsmaßnahme statt, alle Patienten erhielten damit neben der medikamentösen Behandlung ein physiotherapeutisches, ergotherapeutisches und neuropsychologisches Behandlungsprogramm. Die Ergebnisse zeigen bei allen drei Gruppen einen hochsignifikanten Zuwachs an funktioneller Selbständigkeit in der FIM. Nur bei der Imipramingruppe konnte allerdings zwischen den beiden Messzeitpunkten eine signifikante Verbesserung für die Alertness festgestellt werden. Diese Gruppe zeigte auch einen signifikant höheren Benefit in der visuo-räumlichen Suchleistung als die anderen beiden Gruppen. Ein systematischer Zusammenhang zwischen Noradrenalin und visuo-räumlicher Aufmerksamkeitsleistung wird inzwischen durch eine Reihe von neurowissenschaftlichen Studien nahe gelegt. Diese Studie dokumentiert, dass die Behandlung von Patienten mit schweren visuo-räumlichen Aufmerksamkeitsstörungen mit Imipramin, einem Antidepressivum mit erheblichem noradrenergen Wirkanteil, von therapeutischem Nutzen sein könnte.
Drug Influence on Visuospatial Attention Deficit in Patients with Right Hemisphere Media Infarction
21 patients with a right MCA infarction were treated either with imipramin (n = 11) or piracetam (n = 10) and compared with 12 control patients of the same etiology. We compared their performance in visuospatial attention (visual scanning and neglect-test of the attention testing battery), alertness, functional independence (Functional Independence Measure, FIM) and subjective well-being before and after a four weeks treatment. Age and intelligence were matched for the three groups. Treatments were performed in a rehabilitation hospital. All patients received an integrated physiotherapeutic, occupational therapeutic and neuropsychological treatment. The three groups improved significantly in their functional independence. The group treated with imipramin showed a significant increase in the alerting response and a significant better performance in visuospatial attention compared to the two other groups. Recent neuroscientific studies argue for a role of noradrenalin in visuospatial attention and alerting tasks. Our studies show that a treatment of patients who suffer from a deficit in visuospatial attention and in alerting (i.e. patients with right MCA infarcts) with an antidepressant of relevant noradrenergic potency might help to endorse recovery.
