Der chronische Beckenschmerz - Pathophysiologie, Diagnostik und Therapie

Zusammenfassung

Die Diagnostik und Therapie von Patienten mit chronischen Schmerzen im Beckenbereich stellt nach wie vor eine große Herausforderung dar. Neue Einsichten in den pathophysiologischen Hintergrund erlauben ein beginnendes Verständnis für die Entwicklung und Unterhaltung von chronischen Schmerzen. Von einer Aufklärung des Pathomechanismus ist man jedoch noch weit entfernt. Die diagnostischen Bemühungen zielen in erster Linie auf eine Charakterisierung von Funktionsstörungen im Beckenbereich ab, die eine Ursache, aber auch Folge der Schmerzproblematik sein können. Für das therapeutische Procedere gibt es noch keine allgemein akzeptierten Konzepte.

In der vorliegenden Arbeit wird auf die Gesamtproblematik detailliert eingegangen und anhand eigener Erfahrungen und der aktuellen Literatur werden Empfehlungen für ein stufenweises Vorgehen im klinischen Alltag gegeben.

Abstract

Diagnostics and therapy of chronic pelvic pain remains a challenge. New insights in pathophysiology have allowed for a growing understanding of the genesis and maintenance of chronic pain. Diagnostics include efforts to characterize the functional disorders in the pelvic region which can be a cause or a consequence of chronic pelvic pain. There are no generally accepted guidelines for therapy.

This review reports on the pathophysiology, diagnostics and therapy of chronic pelvic pain based on own experiences and the available scientific literature.
