Zusammenfassung
Die Übersichtsarbeit beschreibt eine Klassifikation der Knieendoprothesen, die Indikationsstellung zur zementierten bzw. zementfreien Implantation und die Problematik des Patellaersatzes. Es werden auch die Probleme bei Revisionen und die Nachbehandlungskonzepte besprochen. Die Antwort auf die Frage, ob in der Traumatologie ein primärer oder sekundärer Kniegelenksersatz erfolgen soll, ergibt, dass nach Durchsicht der Literatur und nach den eigenen Erfahrungen dem sekundären Kniegelenksersatz der Vorzug gegeben werden soll. Es kann dadurch die Komplikationsrate gesenkt und die Rehabilitationszeit verkürzt werden. Im Einzelfall wird die selektive Indikation zur primären Knieendoprothese gestellt.
Prosthetic knee replacement in trauma orthopaedics
Summary
The survey publication describes a classification of knee arthroplasties, the indication of a cemented respectively a cementless prosthesis and the problems of patella replacement are also a topic. Problems of revisions and concepts of after-treatment are also discussed. The answer of the question, if a primary or a secondary replacement of the knee joint in traumatology should be performed, is, that we prefer the secondary replacement after studying the literature and because of our own experiences. Therefore the complication rate can be reduced and the time of rehabilitation shortened. Only in some cases we suggest a selective indication of a primary knee prosthesis.
Résumé
Ce travail donne une vue d'ensemble de la classification des endoprothèses, en outre il décrit la position de l'indication par rapport à l'implantation cimentée ou bien nonciment'e et la difficult' du remplacement de la patelle. De plus, cet article contient la problématique lors des révisions et des concepts du traitement complémentaire. La réponse à la question si l'on doit faire un remplacement primaire ou secondaire de l'articulation du genou, c'est qu'on doit donner la préférence à la substitution secondaire de l'articulation du genou. De cette manière on peut baisser le risque des complications et réduire la période de rééducation. En cas isolé on met d'une manière conjointe l'indication sélective et l'endoprothèse du genou.
Resumen
El trabajo que da una vision general, describe una classificación de la endoprótesis de la rodilla, la posición de indicación para la impiantatión cementada o libre de cemento y la problemática de la sustitución de la rótula. Además se habla de los problemas en relación con revisiones y los conceptos para el postoperatorio. La respuesta a la pregunta, si en la traumatología se debe efectuar una primaria o secundaria sustitución de la articulación de la rodilla, es, que segú las propias experiencias y después de revisar la literatura hay que dar preferencia a la secundaria sustitución de la articulación de la rodilla. Asi se puede bajar la tasa de complicaciones el tiempo de rehabilitación se abrevia. En casos particulares se da la indicación selectiva para la endoprótesis de la rodilla.
