Zusammenfassung
Einleitung: Unter den Entwicklungsstörungen der Niere nehmen Fusionsanomalien einen bedeutenden Raum ein. Die häufigste Erscheinungsform stellt dabei die lumbal dystope Hufeisenniere dar, deren untere Pole fusioniert sind. Beckennieren sind demgegenüber wesentlich seltener anzutreffen. Eine frühzeitige, nahezu vollständige Verschmelzung beider Nierenblasteme führt zu einer Kuchen- oder Klumpenniere. Neben der Fusionsanomalie und Dystopie sind diese Organe immer durch eine Rotationsanomalie gekennzeichnet.
Kasuistik: Im vorliegenden Beitrag wird die Fusionsanomalie einer Beckenniere geschildert, die während des Präparierkurses an einem weiblichen Leichnam beobachtet wurde. Dabei stellte die atypische, nicht polverbindende Lokalisation der Parenchymbrücke, sowie aberrante Gefäßverbindungen die eigentlichen Besonderheiten dar. Der morphologisch als „Hantelniere” (Ren halteriformis) bezeichnete Sachverhalt wird als inkomplette Ausprägung einer Kuchen- bzw. Klumpenniere interpretiert.
Schlussfolgerung: Die Komplexität der Nierenentwicklung führt zu einer morphologischen Vielfalt von Entwicklungsstörungen, die sich hinsichtlich Art und Ausprägungsgrad der Fusions-, Rotations- und Lageanomalie des Organs unterscheiden. Neben atypischer Gefäßversorgung und Harnableitung stellt insbesondere die veränderte, variantenreiche Topographie eine Herausforderung für Urologen, Radiologen und Gefäßchirurgen dar. Da diese Gruppe von Entwicklungsstörungen mitunter über lange Zeit asymptomatisch bestehen kann und die Diagnose somit unbekannt bleibt, sollte bei atypischer Symptomatik oder unklarer retroperitonealer Raumforderung auch an eine Nierendystopie gedacht werden.
Abstract
Introduction: Fusion anomalies play an important role in patients with renal malformations. The most frequent manifestation is the lumbar ectopic horseshoe kidney, normally characterized by fused lower renal poles. On the other hand, pelvic kidneys are very rare. Early, extensive fusion of both renal blastema gives rise to a cake or lump kidney. Besides dystopia and failure of separation, fused kidneys are characterized by a rotation anomaly.
Case Report: This case report describes an unusual fusion anomaly of a pelvic kidney found in a female corpse during a dissection course. The remarkable position of the renal isthmus, which did not connect the poles, and the atypical blood supply are the specific features of the anomaly. The morphologically termed „dumbbell” kidney (Ren halteriformis) is considered to be an incomplete manifestation of a cake or lump kidney.
Conclusions: The complexity of renal development leads to a great variety of morphological anomalies which differ in type, location and extent of fusion or rotation, as in dystopia. Besides atypical blood supply and urine drainage, the aberrant topography in particular makes specific demands on urologists, radiologists and vascular surgeons. Due to a longterm asymptomatic clinical course, fusion anomalies are frequently not diagnosed. It is therefore advisable to remember this malformation in the event of indistinct abdominal symptoms or retroperitoneal tumor.
